xen/kexec: Style fixes
Adjust kexec and kimage to more closely adhere to Xen style. Sort the includes, dropping duplicates (kexec.h) and unused (ctype.h and kernel.h). No functional change. Signed-off-by: Andrew Cooper <andrew.cooper3@citrix.com> Acked-by: Roger Pau Monné <roger.pau@citrix.com>
