As of commit d155e4aef3 ("xen: Allow EXPERT mode to be selected from
the menuconfig directly") EXPERT is a regular config option (which the
shim default config also enables).

Some intermediate files weren't considered at all at the time. Also
after its introduction, various changes to the build environment have
rendered the exclusion sets stale. For example, we now have some .*.cmd
files in the build tree. Combine all respective patterns into a single
.* one, seeing that we don't have any actual source files matching this
pattern in the tree. Add other patterns as well as individual files.
Also introduce LINK_EXCLUDE_PATHS to deal with entire directories full
of generated headers as well as a few specific files the names of which
are too generic to list under LINK_EXCLUDES.

Sander reported a build failure which manifests as `make; make install`
failing with:
<snip>/cross-install -m0644 -p xen-dir/xen-shim //usr/local/lib/xen/boot/xen-shim
install: cannot stat 'xen-dir/xen-shim': No such file or directory
make[4]: *** [Makefile:52: install] Error 1
make[4]: Leaving directory '/usr/src/new/xen-unstable/tools/firmware'
It has subsequently been seen intermittently by OSSTest. This was caused by
c/s 32b1d628 triggering a preexisting linkfarm bug for partial rebuilds.
Between the first `make` and the subsequent `make install`, the linkfarm logic
observes new final build products and regenerates the linkfarm. This includes
a distclean, which throws away everything from the first `make`.
As the xen-shim rule use a symlink, the link itself remains still up-to-date
but is broken due to the distclean, which causes install to fail.
Update the linkfarm logic to not regenerate itself when build artefacts
appear. This isn't a comprehensive fix but is the best which can be done
easily. Any further effort would be better spent making out-of-tree builds
work for Xen.

Select MEM_ACCESS_ALWAYS_ON on x86 to mark that MEM_ACCESS is not
configurable on x86. Avoid selecting it on ARM.
Rename HAS_MEM_ACCESS to MEM_ACCESS everywhere. Add a prompt and a
description to MEM_ACCESS in xen/common/Kconfig.
The result is that the user-visible option is MEM_ACCESS, and it is
configurable only on ARM (disabled by default). At the moment the
arch-specific mem_access code remains enabled on ARM, even with
MEM_ACCESS=y.
The purpose is to reduce code size. The option doesn't depend on EXPERT
because it would be nice to ecurity-support configurations without
MEM_ACCESS and a non-expert should be able to disable it.

Changes in v5:
- change MEM_ACCESS_ALWAYS_ON to bool
- change default for MEM_ACCESS, default y if MEM_ACCESS_ALWAYS_ON
Changes in v4:
- remove HAS_MEM_ACCESS
- move MEM_ACCESS_ALWAYS_ON to common
- combile default and bool to def_bool
Changes in v3:
- keep HAS_MEM_ACCESS to mark that an arch can do MEM_ACCESS
- introduce MEM_ACCESS_ALWAYS_ON
- the main MEM_ACCESS option is in xen/common/Kconfig
Changes in v2:
- patch added
Currently building the shim will modify shim.config in case some config
option was added or modified in the hypervisor.
Avoid that by copying shim.config to an intermediate file instead.

There's no need to have shim specific targets, so just use the regular
xen makefile targets in order to build the shim binary.
When the shim is build as part of the firmware directory install the
stripped Xen binary to the firmware directory and place a binary with
symbols in the debug directory.
The objcopy step of the shim build is also removed in this patch:
since the shim is booted in PVH mode there's no need for the resulting
binary to be in elf32 format. Xen can load PVH kernels with either a
32 or 64bit elf header.

"mkdir -p" reports a missing operand, as config/ has no subdirs. Oddly
enough this doesn't cause the whole command (and hence the build to
fail), despite the "set -e" now covering the entire set of commands -
perhaps a quirk of the relatively old bash I've seen this with (a few
simple experiments suggest that commands inside () producing a non-
success status would exit the inner shell, but not the outer one).
Add a dummy . argument to the invocation.
