More NMI-handling cleanups. Users now have an option of what to do
with an error NMI by specifying 'nmi=xxx' on Xen's cmdline.
'nmi=dom0' -> Tell DOM0 about it (DOM0 currently ignores it).
'nmi=ignore' -> ignore it
'nmi=fatal' -> print a diagnostic and then hang.
