The bootloader may call libxl__file_reference_map(), which mmap's the
pv_kernel and pv_ramdisk into process memory. This was only unmapped,
however, on the success path of libxl__build_pv(). If there were a
failure anywhere between libxl_bootloader.c:parse_bootloader_result()
and the end of libxl__build_pv(), the calls to
libxl__file_reference_unmap() would be skipped, leaking the mapped
virtual memory.
Ideally this would be fixed by adding the unmap calls to the
destruction path for libxl__domain_build_state. Unfortunately the
lifetime of the libxl__domain_build_state is opaque, and it doesn't
have a proper destruction path. But, the only thing in it that isn't
from the gc are these bootloader references, and they are only ever
set for one libxl__domain_build_state, the one which is
libxl__domain_create_state.build_state.
So we can clean up in the exit path from libxl__domain_create_*, which
always comes through domcreate_complete.
Remove the now-redundant unmaps in libxl__build_pv's success path.

The fd passed to us by libvirt for both save and restore has at least
O_NONBLOCK set, which libxl does not expect and therefore fails to
handle any EAGAIN which might arise.
This has been observed with migration v2, but if v1 used to work I
think that would be just be by luck and/or coincidence.
Unix convention (and the principal of least surprise) is usually to
ensure that an fd has no "strange" properties, such as being
non-blocking, when handing it to another component.
However for the convenience of the application arrange instead for
libxl to clear any unexpected flags on the file descriptors it is
given for save or restore and restore them to their original state at
the end. O_NDELAY could be similarly problematic so clear that as
well as O_NONBLOCK.
To do this introduce a pair of new helper functions one to modify+save
the flags and another to restore them and call them in the appropriate
places.
The migration v1 code appeared to do some things with O_NONBLOCK in
the checkpoint case. Migration v2 doesn't seem to do so, and in any
case I wouldn't expect it to be relying on libvirt's setting of
O_NONBLOCK when xl doesn't use that flag.

While building a VM, HVM domain builder provides struct hvm_info_table{}
to help hvmloader. Currently it includes two fields to construct guest
e820 table by hvmloader, low_mem_pgend and high_mem_pgend. So we should
check them to fix any conflict with RDM.
RMRR can reside in address space beyond 4G theoretically, but we never
see this in real world. So in order to avoid breaking highmem layout
we don't solve highmem conflict. Note this means highmem rmrr could still
be supported if no conflict.
But in the case of lowmem, RMRR probably scatter the whole RAM space.
Especially multiple RMRR entries would worsen this to lead a complicated
memory layout. And then its hard to extend hvm_info_table{} to work
hvmloader out. So here we're trying to figure out a simple solution to
avoid breaking existing layout. So when a conflict occurs,
#1. Above a predefined boundary (2G)
- move lowmem_end below reserved region to solve conflict;
#2. Below a predefined boundary (2G)
- Check strict/relaxed policy.
"strict" policy leads to fail libxl. Note when both policies
are specified on a given region, 'strict' is always preferred.
"relaxed" policy issue a warning message and also mask this entry INVALID
to indicate we shouldn't expose this entry to hvmloader.
Note later we need to provide a parameter to set that predefined boundary
dynamically.

v13: Mechanical changes to deal with changes to patch 01/
XENMEM_reserved_device_memory_map.
This patch introduces user configurable parameters to specify RDM
resource and according policies,
Global RDM parameter:
rdm = "strategy=host,policy=strict/relaxed"
Per-device RDM parameter:
pci = [ 'sbdf, rdm_policy=strict/relaxed' ]
Global RDM parameter, "strategy", allows user to specify reserved regions
explicitly, Currently, using 'host' to include all reserved regions reported
on this platform which is good to handle hotplug scenario. In the future
this parameter may be further extended to allow specifying random regions,
e.g. even those belonging to another platform as a preparation for live
migration with passthrough devices. By default this isn't set so we don't
check all rdms. Instead, we just check rdm specific to a given device if
you're assigning this kind of device. Note this option is not recommended
unless you can make sure any conflict does exist.
'strict/relaxed' policy decides how to handle conflict when reserving RDM
regions in pfn space. If conflict exists, 'strict' means an immediate error
so VM can't keep running, while 'relaxed' allows moving forward with a
warning message thrown out.
Default per-device RDM policy is same as default global RDM policy as being
'relaxed'. And the per-device policy would override the global policy like
others.

which may not be the layout the user wanted. This allows you to
increase the below 4G address space that PCI devices can use and
therefore in more cases not have any mmio that is above 4G.
There are real PCI cards that do not support mmio over 4G, so if you
want to emulate them precisely, you may also need to increase the
space below 4G for them. There are drivers for these cards that also
do not work if they have their mmio space mapped above 4G.

destruction races with other functions that try to get hold of the lock.
There are several issues:
1. The lock is released too early with libxl__userdata_destroyall
deletes everything in userdata store, including the lock file.
2. The check of domain existence is only done at the beginning of lock
function, by the time the lock is acquired, the domain might have
been gone already.
The effect of this two issues is we can run into such situation:

destroy all files (lock file deleted,

acquire lock (file created)
# LOCK FUNCTION destroy domain

This commit changes the existing libxl code to be sure to grant access
permission to PCI-related I/O memory ranges, while setting up passthrough
of PCI devices specified in the domain's configuration, and to VGA-related
memory ranges, while setting up VGA passthrough (if gfx_passthru = 1 in
the domain's configuration).
As for the latter, the newly-added code does not replace any existing one,
but instead matches the calls to xc_domain_memory_mapping() performed by
QEMU on the path that is executed if gfx passthru is enabled and follows
the registration of a new VGA controller (in register_vga_regions(),
defined in hw/pt-graphics.c). In fact, VGA needs some extra memory
ranges to be mapped with respect to PCI; QEMU expects that access to those
memory ranges is implicitly granted when he calls the hypervisor with the
function xc_domain_memory_mapping(): this commit calls iomem_permission
for it when needed by checking the passthru PCI device's class.
NOTE: the code added by this commit still does not verify if the passthru
of the framebuffer area is being performed for the primary GPU, but
only replicates the behavior of QEMU which is limited to performing
the passthru for all PCI devices of VGA class.
This commit is instrumental to the last one in the series, which will
separate the functions of the iomem_permission and memory_mapping DOMCTLs,
so that requesting an I/O-memory range will not imply that access to such
a range is implicitly granted.

The configuration is stored in libxl-json format. It will be used as
template to reconstruct domain configuration during runtime.
There's only one write to disk when domain creation finishes. We
therefore have a window that the domain exists but has no JSON config in
disk. We define this state as domain being created or destroyed. Any
other operations that need to access JSON config should bail.
