The only remaining mentions of these addresses are in COPYING files which I
haven't touched.
Some of the changed files are imports from elsewhere, however
filtering them out is tricky, I think it is tolerable to have these
files be modified here and then perhaps reverted on the next sync,
since it's only 1-2 lines and obvious what is going on.

With hvmloader telling the guest that it may skip REG_C reads during
the processing of RTC interrupts, the emulation code must not depend
upon these reads to occur. Introduce two modes of operation for the
emulation code, and short of a HVM parameter (too late to be
introduced for 4.3) hard code the mode determination to always assume
that Windows-conforming one for the time being.

When booting a Windows guest, the OS report an issue with the ACPI (in
a BSOD). The exact issue is "SCI_EN never becomes set in PM1 Control
Register." (quoted from WinDbg help).
So this patch enables the flags SCI_EN if it is not yet enabled.

...because the default timesource (TSC) drifts under load.
The flag is only defined in ACPI 3.0, and we provide ACPI 2.0 tables,
but Windows seems happy enough to obey it anyway.

This patch adds the ACPI fixed hardware power button for HVM.
It enables a graceful shutdown of a guest OS by direction of Dom0.
(if a proper action for the power button is set inside the guest)

** Currently only 2 virtual hotplug pci slots(6~7) are created so more
than 2 vtd dev can't be hotplugged, but we can easily extend it in
future.
Three new commands are added:
"xm pci-list domid" show the current assigned vtd device, like:
VSlt domain bus slot func
0x6 0x0 0x02 0x00 0x0
"xm pci-detach" hot remove the specified vtd device by the virtual
slot, like:
xm pci-detach EdwinHVMDomainVtd 6
"xm pci-attach DomainID dom bus dev func [vslot]" hot add a new vtd
device in the vslot. If no vslot specified, a free slot will be picked
up. e.g. to insert '0000:03:00.0':
xm pci-attach EdwinHVMDomainVtd 0 3 0 0
** guest pci hotplug

1. Override ISA IRQs 5,6,10,11 as active-low level-triggered.
These are 'wired' to PCI devices.
2. Limit PCI-ISA link routers to route to 5,6,10,11. Others
are active-high edge-triggered and so invalid to use for
PCI devices. In any case, four ISA IRQs is sufficient to
avoid unnecessary aliasing among four links.
Also remove PCI device 0 IRQ info. It's an embedded host bridge with
no interrupt sources.
