vchan/init.c is defining offsetof privately. Remove that definition
and just use stddef.h instead.

Defining min(), min_t(), max() and max_t() at other places than
xen-tools/common-macros.h isn't needed, as the definitions in said
header can be used instead.
Same applies to BUILD_BUG_ON() in hvmloader.

vchan server creates XenStore entries to advertise its event channel and
ring, but those are not removed after the server quits.
Add additional cleanup step, so those are removed, so clients do not try
to connect to a non-existing server.

There is no reason why libvchan is not placed in the tools/libs
directory.
At the same time move libxenvchan.h to a dedicated include directory
in tools/libs/vchan in order to follow the same pattern as the other
libraries in tools/libs.
As tools/libvchan now contains no library any longer rename it to
tools/vchan.
