This patch makes the flexarray function libxl__gc aware.
It also updates every function that use a flexarray to pass the gc and removes
every memory allocation check and free.

It was convenient to invent $(CFLAGS_LIBXL) to do this.
Various renamings to avoid shadowing standard functions:
- index(3)
- listen(2)
- link(2)
- abort(3)
- abs(3)
Reduced the scope of some variables to avoid conflicts.
Change to libxc is due to the nested hypercall buf macros in
set_xen_guest_handle (used in libxl) using the same local private vars.
Build tested only.

Introduce flexarray_append_pair: a simple syntactic sugar to add a pair
of pointers to a flexarray, very useful when adding pairs of strings to
xenstore.
Replace flexarray_vappend calls (and one pair of flexarray_append)
with flexarray_append_pair calls when dealing with flexarrays that are
going to be used with libxl__xs_kvs_of_flexarray:
NULL is a valid pointer value in these cases while flexarray_vappend
uses NULL as vargs terminator, so the old code is buggy.

This kills off about 1,000 PLT entries speeding up link time and shaving about
1-2KB off of binary size. It also prevents users of the library erroneously
calling libxl internal functions.
