ewan@localhost.localdomain e83be11595 Move all internal header files to a separate directory, so that it's clear what 
forms the public API.  Tidy up a couple of macros in xen_internal.h.  Remove
test_hvm_bindings, which no longer builds, and which has had its functionality
subsumed into test_bindings.  Bump the library version to 0.9.

Ewan Mellor 9b91aeb6ad Replace uses of the typename (e.g. xen_host) where its used as a variable name. 
This is an unusual style, and out of sync with the rest of the library.

Ewan Mellor 07b95037c7 Fix the handling of sets in the interface -- expecting a pointer to X to serve 
as a list of them was too hopeful -- that's what the set structures are for.
