Begin to untangle the header dependency tangle by moving definition of
struct cpuid_leaf out of x86_emulate.h into the new cpuid.h.
Additionally, plumb the header through to libxc. This is technically a
redundant include at this point, but it helps build-test the later changes,
and will be used eventually.

Running "make uninstall" does not remove all installed files, a
situation which might cause link related issues if xen is re-installed
in a different location.
In order to make uninstall correctly remove the files it is best
the process should be done recursively by mirroring each "install"
target with an "uninstall" who removes the installed files.
An exception to this rule is uninstalling the files produced by
"qemu-xen-dir-remote" and "qemu-xen-traditional-dir", which are external
to the project. These projects do not implement an "uninstall" target so
the files have to be removed manually.

Build can fail if stubdom build is run before tools build because:
1. tools/include build uses relative path and depends on XEN_OS
2. stubdom needs tools/include to be built, at which time XEN_OS is
mini-os and corresponding symlinks are created
3. libraries inside tools needs tools/include to be built, at which
time XEN_OS is the host os name, but symlinks won't be created
because they are already there
4. libraries get the wrong headers and fail to build
Since both tools and stubdom build need the public headers, we build
tools/include before stubdom and tools. Remove runes in stubdom and
tools to avoid building tools/include more than once.
Provide a new dist target for tools/include. Hook up the install,
clean, dist and distclean targets for tools/include.
The new arrangement ensures tools build gets the correct headers
because XEN_OS is set to host os when building tools/include. As for
stubdom, it explicitly links to the mini-os directory without relying
on XEN_OS so it should be fine.

The link to acpi include directory is not removed by Makefile's 'clean'
target. This can lead to make failure when making xen/.dir target if
we try to create the link again.
We can prevent this failure by (1) removing acpi link when cleaning up
and (2) adding '-f' option to 'ln' (just like we do for other targets).
We should also add tools/include/acpi link to .gitignore.

Do not include ../../xen/include/ to build libxl_arm_acpi.c: header
files clashing against default headers under /usr/include are present in
that directory.
Link only $(XEN_ROOT)/xen/include/acpi under tools/include instead.

The original rule to target "distclean" in tools/Rules.mk was in effect
"make clean". It should be "make distclean".
However not all Makefiles in subdirectories have distclean target
defined. So this patch also adds a bunch of distclean targets to various
Makefiles. They only depend on clean target and don't have any actions
in most cases.
With the patch applied, following command outputs 0 results:
find tools -name 'Makefile*' -exec grep -L 'distclean' {} \+ \
| grep -v ocaml | grep -v libfsimage
Ocaml and libfsimage are known to have distclean defined in a dedicated
rules file.
