A previous patch added support for priv-mapping guest resources directly
(rather than having to foreign-map, which requires P2M modification for
HVM guests).
This patch makes use of the new API to seed the guest grant table unless
the underlying infrastructure (i.e. privcmd) doesn't support it, in which
case the old scheme is used.
NOTE: The call to xc_dom_gnttab_hvm_seed() in hvm_build_set_params() was
actually unnecessary, as the grant table has already been seeded
by a prior call to xc_dom_gnttab_init() made by libxl__build_dom().

Recent changes in grant table configuration have caused calls to

3rd party dombuilder users.
Add max_{grant,maptrack}_frames parameters to struct xc_dom_image, and require
them to be set by callers using xc_dom_gnttab_init(). Libxl, which uses
xc_dom_gnttab_init() itself is updated appropriately.

libxl always uses xc_dom_gnttab_init(), which internally calls
xc_dom_gnttab{_hvm,}_seed() to set up the grants point at the console and
xenstore rings. For HVM guests, libxl then asks Xen for the information set
up previously, and calls xc_dom_gnttab_hvm_seed() a second time, which is
wasteful. ARM construction expects libxl to have set up
dom->{console,xenstore}_evtchn earlier, so only actually functions because of
this second call.
Rationalise everything and make it consistent for all guests.
1) Users of the domain builder are expected to provide
dom->{console,xenstore}_{evtchn,domid} unconditionally. This is checked
by setting invalid values in xc_dom_allocate(), and checking in
xc_dom_boot_image().
2) For x86 HVM and ARM guests, the event channels are given to Xen at the
same time as the ring gfns. ARM already did this, but x86 is updated to
match. x86 PV already provides this information in the start_info page.
3) Libxl is updated to drop all relevant functionality from
hvm_build_set_params(), and behave consistently with PV guests when it
comes to the handling of dom->{console,xenstore}_{evtchn,domid,gfn}.
This removes several redundant hypercalls (including a foreign mapping) from
the x86 HVM and ARM construction paths.

The sole use of xc_dom_translated() and xc_dom_p2m() outside of the domain

into useful values. PV guest pfns are only interesting to the domain builder,
and gfns are the address space used by all other hypercalls.
Renaming the fields in xc_dom_image is deliberate, as it will cause
out-of-tree users of the dombuilder to notice the different semantics.
Correct the terminology throughout xc_dom_gnttab{_hvm,}_seed(), which are all

pfn 0 is a legitimate (albeit unlikely) frame to use, so skipping it is wrong.
This behaviour appears to exists simply to cover the fact that zero is the
default value of an uninitialised field in dom.
ARM already clears the frames at the point that the pfns are allocated,
meaning that the added clear_page() is wasteful. Alter x86 to match ARM and
clear the page when it is allocated.

The code to initialize the grant table in libxc uses
xc_domain_maximum_gpfn() + 1 to get a guest pfn for mapping the grant
frame and to initialize it.
This solution has two major issues:
- The check of the return of xc_domain_maximum_gpfn is buggy because
xen_pfn_t is unsigned and in case of an error -ERRNO is returned.
Which is never catch with ( pfn <= 0 ).
- The guest memory layout maybe filled up to the end, i.e
xc_domain_maximum_gpfn() + 1 gives either 0 or an invalid PFN due to
hardware limitation.
Futhermore, on ARM, xc_domain_maximum_gpfn() is not implemented and
return -ENOSYS. This will make libxc to use always the same PFN which
may colapse with an already mapped region (see xen/include/public/arch-arm.h
for the layout).
This patch only address the problem for ARM, the x86 version use the same
behavior (ie xc_domain_maximum_gpfn() + 1), as I'm not familiar with Xen x86.
A new function xc_core_arch_get_scratch_gpfn is introduced to be able to
choose the gpfn per architecture.
For the ARM version, we use the GUEST_GNTTAB_GUEST which is the base of
the region by the guest to map the grant table. At the build time,
nothing is mapped there.
At the same time correctly check the return of xc_domain_maximum_gpfn
for x86.

Guests are initially started with caches disabled and so we need to make sure
they see consistent data in RAM (requiring a cache clean) but also that they
do not have old stale data suddenly appear in the caches when they enable
their caches (requiring the invalidate).
This can be split into two halves. First we must flush each page as it is
allocated to the guest. It is not sufficient to do the flush at scrub time
since this will miss pages which are ballooned out by the guest (where the
guest must scrub if it cares about not leaking the pagecontent). We need to
clean as well as invalidate to make sure that any scrubbing which has occured
gets committed to real RAM. To achieve this add a new cacheflush_page function,
which is a stub on x86.
Secondly we need to flush anything which the domain builder touches, which we
do via a new domctl.

In order to set up the grant table for HVM guests, libxc needs to map
the grant table temporarily. At the moment, it does this by adding the
grant page to the HVM guest's p2m table in the MMIO hole (at gfn 0xFFFFE),
then mapping that gfn, setting up the table, then unmapping the gfn and
removing it from the p2m table.
This breaks with PVH guests with 4G or more of ram, because there is
no MMIO hole; so it ends up clobbering a valid RAM p2m entry, then
leaving a "hole" when it removes the grant map from the p2m table.
Since the guest thinks this is normal ram, when it maps it and tries
to access the page, it crashes.
This patch maps the page at max_gfn+1 instead.

This patch adds the option "superpages" to the domain configuration
file. If it is set, the domain is populated using 2M pages.
This code does not support fallback to small pages. If the domain can
not be created with 2M pages, the create will fail.
The patch also includes support for saving and restoring domains with
the superpage flag set. However, if a domain has freed small pages
within its physical page array and then extended the array, the
restore will fill in those freed pages. It will then attempt to
allocate more than its memory limit and will fail. This is
significant because apparently Linux does this during boot, thus a
freshly booted Linux image can not be saved and restored successfully.
