Replace the firmware loaded into HVM guests with an OS kernel. Since the HVM
builder now uses the PV xc_dom_* set of functions this kernel will be parsed
and loaded inside the guest like on PV, but the container is a pure HVM
guest.
Also, if device_model_version is set to none or a device model for the
specified domain is not present unconditinally set the nic type to
LIBXL_NIC_TYPE_VIF.

A platform may have a GIC compatible with previous version of the
device.
This is allow to virtualize an unmodified OS on new hardware if the GIC
is compatible with older version.
When a guest is created, the vGIC will emulate same version as the
hardware. Although, the user can specify in the configuration file the
preferred version (currently only GICv2 and GICv3 are supported).

ARM and x86 use a different hypercall to map an IRQ to a domain.
The hypercall to give IRQ permission to the domain has also been moved
to be an x86 specific function as ARM guest won't be able to manage the IRQ.
We may want to support it later.

On ARM the virtual GIC may differ between each guest (emulated GIC version,
number of SPIs...). This information is already known at the domain creation
and can never change.
For now only the gic_version is set. In the long run, there will be more
parameters such as the number of SPIs. All will be required to be set at the
same time.
A new arch-specific structure arch_domainconfig has been created, the x86
one doesn't have any specific configuration, for now, a dummy structure
(C-spec compliant) has been created.
Some external tools (qemu, xenstore) may be required to create a domain.
Rather than asking them to take care of the arch-specific domain
configuration, let the current function (xc_domain_create) chose a
default configuration and introduce a new one (xc_domain_create_config).
This patch also drops the previously introduced DOMCTL arm_configure_domain
in Xen 4.5, as it has been made useless.

Introduce a arch-independent routine to generate one vmemrange per
vnode. Also introduce arch-dependent routines for different
architectures because part of the process is arch-specific -- ARM has
yet have NUMA support and E820 is x86 only.
For those x86 guests who care about machine E820 map (i.e. with
e820_host=1), vnode is further split into several vmemranges to
accommodate memory holes. A few stubs for libxl_arm.c are created.

The ramdisk is passed to the kernel as a property in the chosen node of the
device tree. This is somewhat tricky since in order to place the ramdisk and
dtb in ram we first need to know the size of the dtb. So we initially create a
DTB with placeholders for the ramdisk and finalise the value (which doesn't
change the size) once we know where everything is.
Rename libxl__arch_domain_configure to xl__arch_domain_init_hw_description to
better reflect its use and to be consistent with the new
libxl__arch_domain_finalise_hw_description.
The common xc_dom_build_image() function did not support explicit placement of
the ramdisk, instead passing 0 to xc_dom_alloc_segment, meaning "pick
somewhere". This change instead passes ramdisk_seg.vstart. If nothing has set
vstart then it will be zero because the entire dom struct is zeroed on
allocation in xc_dom_allocate(). Therefore there is no change to the behaviour
on x86. This is also consistent with how other segments (kernel, dtb) are
handled.
Furthermore if the ramdisk has been explicitly placed then xc_dom_build_image()
assumes that it is not to be decompressed (since that would muck up the sizings
used on placement).
With all that I'm able to boot a domain using the current Debian Jessie armhf
installer initrd and have it complete successfully.

Uses xc_dom_devicetree_mem which was just added. The call to this needs to be
carefully sequenced to be after xc_dom_parse_image (so we can tell which kind
of guest we are building, although we don't use this yet) and before
xc_dom_mem_init which tries to decide where to place the FDT in guest RAM.
Removes libxl_noarch which would only have been used by IA64 after this
change. Remove IA64 as part of this patch.
There is no attempt to expose this as a configuration setting for the user.
Includes a debug hook to dump the dtb to a file for inspection.

The code had an obvious bug where it would assume that the balloon
amount would always be _something_ and add an E820_RAM entry at the
end of the E820 array. The added E820_RAM would contain the balloon amount
plus the delta of memory that had to be subtracted b/c of the various
E820 entries. That assumption is certainly true when maxmem != mem,
but if guest config has maxmem = memory that is incorrect (as balloon
value is zero). The end result is that the E820 that is constructed
is missing a swath of "delta" memory and in most cases ends up with
only one E820_RAM entry that is of 512MB size on many Intel systems.
