This sets a saner default for the cpu-online-map by setting all bits
to 1. The default assumption ought to be that nr-vcpus ==
nr-vcpus-at-start. If that is not true, then the toolstack must modify
the bitmap, but if it is true, the toolstack oughtn't need to do
anything further.

Currently, cur_pages (which is used as index into page_array for
fetching gfns) is used to judge whether it is proper here to allocated
1G pages. However, cur_pages == page_array[cur_pages] only holds true
when it is below 4G. When it is above 4G, page_array[cur_pages] -
cur_pages = 256M.
As a result, when guest has 10G memory, 8 1G-pages are allocated. But
only 2 of them have their corresponding gfns 1G aligned. The other 6
are forced to split to 2M pages, as their starting gfns are 4G+256M,
5G+256M .................
Inside the patch, true gfns are used instead of cur_pages to fix this
issue.

Currently the wrappers for these hypercalls swallow partial success
and return failure to the caller.
In order to use these functions more widely instead of open-coding
uses of XENMEM_* and xc_memory_op add variants which return the actual
hypercall result.
Therefore add the following functions:
xc_domain_increase_reservation
xc_domain_decrease_reservation
xc_domain_populate_physmap
and implement the existing semantics using these new functions as
xc_domain_increase_reservation_exact
xc_domain_decrease_reservation_exact
xc_domain_populate_physmap_exact
replacing the existing xc_domain_memory_* functions.
Use these new functions to replace all open coded uses of
XENMEM_increase_reservation, XENMEM_decrease_reservation and
XENMEM_populate_physmap.
Also rename xc_domain_memory_*_pod_target to xc_domain_*_pod_target
for consistency.
Temporarily add a compatibility macro for
xc_domain_memory_populate_physmap to allow time for qemu to catch up.

tools/libxc, tools/libelf: Relicense under LGPL v2.1
Relicense these two libraries under LGPL v2.1 only except where
individual files already included the "or later" provision.
Copyright holders have been contacted by Stephen Spector and have all
agreed this change.
Removed tools/libxc/ia64/aclinux.h since it appeared to be
unused. There is a separate, more up to date, copy in
xen/include/acpi/platform/aclinux.h which does appear to be used.
Clarify the license of MiniOS privcmd.h under the same terms as other
tools/include/xen-sys headers.

If libelf calls elf_err() or elf_msg() before elf_set_log() has been
called then it could potentially read an uninitialised log handling
callback function pointer from struct elf_binary. Fix this in libxc by
zeroing the structure before calling elf_init().

This patch eliminate the global variables in libxenctrl (used for
logging and error reporting).
Instead the information which was in the global variables is now in a
new xc_interface* opaque structure, which xc_interface open returns
instead of the raw file descriptor; furthermore, logging is done via
xentoollog.
There are three new parameters to xc_interface_open to control the
logging, but existing callers can just pass "0" for all three to get
the old behaviour.
All libxc callers have been adjusted accordingly.
Also update QEMU_TAG for corresponding qemu change.

Implement libxc interface to PoD functionality:
* Add xc_hvm_build_target_mem(), which takes both memsize and target.
Memsize is the total memory, allocated in PoD pages and reported in
the e820; target is the size of the cache. If these are the same, the
normal funcitonality is called. (So you can use the same function to
build always, and it will decide whether to use PoD or not.)
* Add xc_domain_memory_[gs]et_pod_target(), which sets and/or returns
information about the PoD cache and p2m entries.

We use the EPT page table as P2M (guest physical to machine
mapping), removing the linear page table when EPT is used for the
domain (see the new file p2m-ept.c). We did this by adding three
operations in the p2m_domain. If VT-d is enabled, the EPT page table
will be used as the VT-d page table as well (i.e. shared).

Instead of kludging a max_gpfn estimate in shared_info, add a new
XENMEM command to discover the actual maximum gpfn value as known by
the shadow code.
This needs to be more robust when we support HVM ballooning in future
anyway. One interesting point is that max_gpfn may be close to 4GB
even for small-memory HVM guests since for example SVGA LFB is mapped
into the I/O hole. We may need to special case the I/O hole somehow,
or provide some finer-grained way to find out which parts of the GPFN
space are actually used (e.g., get Xen to fill in a bitmap with 1 bit
per 1024 pages, or similar).

- An 'xc_error' struct is used to pass around error
details. Currently contains two members 'code' an enumeration of
error types, and 'message' a free text description of the specific
problem.
- The xc_get_last_error() method returns a const pointer to the
internal instance of this struct manged by libxc. By returning a
const pointer we can add extra members to the end of the struct at
any time without worrying about ABI of callers. This will let us
provide more fine-grained info if needed in the future.
- The xc_error instance is statically defined inside libxc and marked
__thread. This ensures that errors are recorded per-thread, and
that when dealing with errors we never need to call malloc - all
storage needed is statically allocated.
- The xc_clear_last_error() method resets any currently recorded
error details
- The xc_error_code_to_desc() method converts the integer error code
into a generic user facing messsage. eg "Invalid kernel". Together
with the 'message' field from xc_error, this provides the user
visible feedback. eg "Invalid kernel: Non PAE-kernel on PAE host."
- A callback can be registered with xc_set_error_handler to receive
notification whenever an error is recorded, rather than querying
for error details after the fact with xc_get_last_error
- If built with -DDEBUG set, a default error handler will be
registered which calls fprintf(stderr), thus maintaining current
behaviour of logging errors to stderr during developer builds.
- The python binding for libxc is updated to use xc_get_last_error
to pull out error details whenever appropriate, instead of
returning info based on 'errno'
- The xc_set_error method is private to libxc internals, and is used
for setting error details
- The ERROR and PERROR macros have been updated to call xc_set_error
automatically specifying XC_INTERNAL_ERROR as the error code. This
gives a generic error report for all current failure points
- Some uses of the ERROR macro have been replaced with explicit
calls to xc_set_error to enable finer grained error reporting. In
particular the code dealing with invalid kernel types uses this
to report about PAE/architecture/wordsize mismatches
The patch has been tested by calling xm create against a varietry of
config files defining invalid kernels of various kinds. It has also
been tested with libvirt talking to xend. In both cases the error
messages were propagated all the way back up the stack.
There is only one place where I need to do further work. The suspend
& restore APIs in Xend invoke external helper programs rather than
calling libxc directly. This means that error details are essentially
lost. Since there is already code in XenD which scans STDERR from
these programs I will investigate adapting this to extract actual
error messages from these helpers.
