Thus far mem_access and mem_sharing memops had been able to signal
to Xen to start pulling responses off the corresponding rings. In this patch
we retire these memops and add them to the option to the vm_event_op domctl.
The vm_event_op domctl suboptions are the same for each ring thus we
consolidate them into XEN_VM_EVENT_ENABLE/DISABLE/RESUME.
As part of this patch in libxc we also rename the mem_access_enable/disable
functions to monitor_enable/disable and move them into xc_monitor.c.

The spin-lock implementation in the xen-access test program is implemented
in a fashion that is actually incomplete. The x86 assembly that guarantees that
the lock is held by only one thread lacks the "lock;" instruction.
However, the spin-lock is not actually necessary in xen-access as it is not
multithreaded. The presence of the faulty implementation of the lock in a non-
multithreaded environment is unnecessarily complicated for developers who are
trying to follow this code as a guide in implementing their own applications.
Thus, removing it from the code improves the clarity on the behavior of the
system.
Also converting functions that always return 0 to return to void, and making
the teardown function actually return an error code on error.

In this patch we mechanically rename mem_event to vm_event. This patch
introduces no logic changes to the code. Using the name vm_event better
describes the intended use of this subsystem, which is not limited to memory
events. It can be used for off-loading the decision making logic into helper
applications when encountering various events during a VM's execution.

The public mem_event structures used to communicate with helper applications via

This patch remedies the issue by clearly defining the structure members based on

The "A" constraint, while documented up to gcc 4.5 as "The a and d
registers, as a pair (for instructions that return half the result in
one and half in the other)," never really behaved that (natural) way,
but always meant (and is now also documented so) %eax _or_ %edx (%rax
_or_ %rdx on x86-64) unless the operand was wide enough to require both
(i.e. more than 32 bits on ix86 and more than 64 bits on x86-64).
Interestingly something internal to the compiler changed between 4.4
and 4.5 to actually expose the difference - up to gcc 4.4 I was unable
to construct a case where, when only the low half of the result is
actually looked at, the result would be considered to be in %edx/%rdx
(and %eax/%rax would be treated as unmodified by the instruction).

but also upon whether saved state was actually modified (and register
values are known to have been preserved).
Note that RBP is unconditionally considered a volatile register (i.e.
irrespective of CONFIG_FRAME_POINTER), since the RBP handling would
become overly complicated due to the need to save/restore it on the
compat mode hypercall path [6th argument].
Note further that for compat mode code paths, saving/restoring R8...R15
is entirely unnecessary - we don't allow those guests to enter 64-bit
mode, and hence they have no way of seeing these registers' contents
(and there consequently also is no information leak, except if the
context saving domctl would be considered such).

Per page operations in the paging, sharing, and access tracking subsystems are
all implemented with domctls (e.g. a domctl to evict one page, or to share one
page).
Under heavy load, the domctl path reveals a lack of scalability. The domctl
lock serializes dom0's vcpus in the hypervisor. When performing thousands of
per-page operations on dozens of domains, these vcpus will spin in the
hypervisor. Beyond the aggressive locking, an added inefficiency of blocking
vcpus in the domctl lock is that dom0 is prevented from re-scheduling any of
its other work-starved processes.
We retain the domctl interface for setting up and tearing down
paging/sharing/mem access for a domain. But we migrate all the per page
operations to use the memory_op hypercalls (e.g XENMEM_*).
