1. Balloon driver does not need to set M2P entry.
This is done by the populate_physmap hypercall.
2. Xen now translates foreign mappings from GMFN->MFN.
Tools are simplified because of this.

TODO:
1. Fix IO-APIC ID to not conflict with LAPIC IDS.
2. Fix i8259 device model (seems to work already though!).
3. Add INTSRC overrides in MPBIOS and ACPI tables so
that PCI legacy IRQ routing always ends up at an
IO-APIC input with level trigger. Restricting link
routing to {5,6,10,11} and setting overrides for all
four of those would work.

This simplifies the IRQ logic significantly and avoids the bogus
hvm_pic_assist() on domain resume path.
There is more work to be done here. At least:
1. set-irq-level should really be set-interrupt-wire-level. Wire
state needs to be distinguished from PIC (in particular, PIC IRR)
state.
2. Hypercalls can be batched in qemu and pushed down in one
multicall.

1. platform_op -- used by dom0 kernel to perform actions on the
hardware platform (e.g., MTRR access, microcode update, platform
quirks, ...)
2. domctl -- used by management tools to control a specified domain
3. sysctl -- used by management tools for system-wide actions
Benefits include more sensible factoring of actions to
hypercalls. Also allows tool compatibility to be tracked separately
from the dom0 kernel. The assumption is that it will be easier to
replace libxenctrl, libxenguest and Xen as a matched set if the
dom0 kernel does not need to be replaced too (e.g., because that
would require vendor revalidation).
From here on we hope to maintain dom0 kernel compatibility. This
promise is not extended to tool compatibility beyond the existing
guarantee that compatibility will not be broken within a three-level
stable release [3.0.2, 3.0.3, etc.].

paravirtualized and fully virtualized. This approach uses the new
sched_op to handle other domains than the current domain. The new
code, SCHEDOP_remote_shutdown, is very much like SCHEDOP_shutdown, but
is called with the id of the domain which is to be shut down. This
allows fully virtualized shutdown and para-virtualized shutdown to be
identical from that point forward.
A paravirtualized domain uses sched_op to shut down and set the reason
code. This will send a VIRQ_DOM_EXC, which can be handled in dom0 by
control software. In some ways, this resembles SIGCHILD/waitpid, and
is a reasonable model.
The fully virtualized case has qemu invoke xm directly. This is a
different path than paravirtualized. It also removes decision and
policy making choices from the rest of the control software and places
it within qemu. When any dom0 logic eventually gets a VIRQ_DOM_EXC,
the information about the domain is gone having been destroyed by xm.
A libxenctrl wrapper, xc_shutdown_domain has been added and qemu now
calls it.
As a freebie, #if 0 some very verbose logging code in qemu. Totally
unrelated, but as long as I was there...

output. Refactor the code slightly to allow more user-specified
control. This is done by adding a parallel
xc_domain_dumpcore_via_callback, which allows the specification of a
callback routine and an opaque argument block. The existing dumpcore
routine is modified to use the callback for all write operations and
to turn the single seek into a small write (it's for page alignment).

* xc_ptrace
- Merge xc_ptrace_core into xc_ptrace
- ATTACH now reads the data argument. If non-zero then
a corefile is being debuged. Otherwise a thread has
been attached to. This allows xc_waitdomain_core() or
xc_waitdomain() to be called as appropriate in
subsequent xc_ptrace() calls.
* xc_waitdomain
- Rename xc_waitdomain (xc_ptrace.c version) __xc_waitdomain
- Rename xc_waitdomain (xc_ptrace_core.c version) xc_waitdomain_core
- Create xc_waitdomain (in xc_ptrace.c), a wrapper for __xc_waitdomain
and xc_waitdomain_core.
Consolidation seemed difficult but ctxt needs to be
passed into xc_waitdomain_core or made global.
Alternatively, xc_waitdomain_core could be moved into xc_ptrace.c,
but this seems messy.
* map_domain_va
- Rename map_domain_va (xc_ptrace_core.c version) map_domain_va_core
- Have it accept ctxt, like xc_waitdomain_core
* myptrace and myxcwait (linux-xen-low.c)
Removed, call the now generic xc_ptrace() and xc_waitdomain() instead
When calling xc_ptrace ATTACH, if a corefile is in use, a fd will
be passed, otherwise a pid. The fd part is important, as this
is saved internally in xc_ptrace_core.c, and passed to xc_waitdomain_core()
as neccessary. Pereviously xc_waitdomain_core() received a pid and
thus could not open the corefile.

move the shared info and grant table pfns into that block. This
allows us to remove the get_gnttablist dom0 op, and simplifies the
domain creation code slightly. Having the reserved block managed by
Xen may also make it slightly easier to handle the case where the
grant table needs to be extended at run time.
