The variable isn't used past the loop, and its value also isn't
meaningful across iterations. Reduce its scope to make this more
obvious.

In order to better reflect the contents of the header and to make it
more appropriate to use it for different runtime environments like
programs, libraries, and firmware, rename the libs.h include file to
common-macros.h. Additionally add a comment pointing out the need to be
self-contained.

The last libxc internal used by xen-mfndump.c is the ERROR() macro.
Add a simple definition for that macro to xen-mfndump.c and replace
the libxc private header includes by official ones.

The definition of PAGE_SIZE comes from xc_private.h, which shouldn't be
used by xen-mfndump.c. Replace PAGE_SIZE by XC_PAGE_SIZE, as
xc_private.h contains:
#define PAGE_SIZE XC_PAGE_SIZE
For the same reason PAGE_SHIFT_X86 needs to replaced with
XC_PAGE_SHIFT.

xen-mfndump.c is including the libxc private header xg_save_restore.h.
Avoid that by moving the definition of is_mapped() to xen-mfndump.c
(it is used there only) and by duplicating the definition of
M2P_SIZE() in xen-mfndump.c.

xen-tools/libs.h currently contains a shared BUILD_BUG_ON() implementation and
is used by some tools. Extend this to include ARRAY_SIZE and clean up all the
opencoding.

libxenforeignmemory will provide a stable API and ABI for mapping
foreign domain memory (subject to appropriate privileges).
The new library exposes an interface equivalent to
xc_map_foreign_memory_bulk, which all the other
xc_map_foreign_memory_* functions (which remain in libxc) are
implemented in terms of.
Upon request (via #define XC_WANT_COMPAT_MAP_FOREIGN_API) libxenctrl
will provide a compat API for the old names. This is used by qemu-xen
and qemu-trad as well as various in tree things (which required
de-dupping various #includes in some too to get the #define before the
first).

we return now the return value (or zero for success).
The max_mfn is now passed in as the parameter.

xen-hptool and xen-mfndump include xc_private.h. This is bad, but not trivial
to fix, so they gain a protective #undef and a stern comment.
MiniOS leaks ARRAY_SIZE into the libxc namespace as part of a stubdom build.
Therefore, xc_private.h gains an #ifndef until MiniOS is fixed.

* Don't walk off the end of p2m_table under the mistaken impression that it
contains toolstack unsigned longs. Despite its array type it contains guest
unsigned longs so unconditionally needs casting to the guest width to use
correctly. Furthermore, a 64bit toolstack must be extra careful when it
finds a 32bit guest's INVALID_MFN.
* Drop 'mapped' and 'pinned' descriptions. This are both bogus, including all
uses of the is_mapped() macro.
* Rearrange the type name printing to be more concise.

This is what it can do as of now:

Commands:

dump-p2m <domid> show P2M of <domid>
dump-ptes <domid> <mfn> show the PTEs in <mfn>
lookup-pte <domid> <mfn> find the PTE mapping <mfn>
