Commit cefeffc7e5 marked ACPI tables as NVS in the hvmloader path
because SeaBIOS may otherwise just mark it as RAM. There is, however,
yet another reason to do it even in the PVH path. Xen's incarnation of
AML relies on having access to some ACPI tables (e.g: _STA of Processor
objects relies on reading the processor online bit in its MADT entry)
This is problematic if the OS tries to reclaim ACPI memory for page
tables as it's needed for runtime and can't be reclaimed after the OSPM
is up and running.

vCPU as probed at runtime rather than assuming a predefined relation.
Moved smp_initialise() ahead of apic_setup() in order to initialise
cpu_to_apicid ASAP and avoid using it uninitialised. Note that bringing
up the APs doesn't need the APIC in hvmloader becasue it always runs
virtualized and uses the PV interface.

IBT requires the placement of endbr{32,64} instructions at the target of every
indirect call/jmp, and every entrypoint.
However, the default -fcf-protection=branch places an endbr{32,64} on every
function which far more than necessary, and reduces the quantity of protection
afforded. Therefore, we use manual placement using the cf_check attribute.

has uncovered multiple issues:
1) The userspace hypercall wrappers truncate all return values to int on

The current build of the firmware relies on having 32bit compatible
headers installed in order to build some of the 32bit firmware.
Usually this can be solved by using the -ffreestanding compiler option
which drops the usage of the system headers in favor of a private set
of freestanding headers provided by the compiler itself that are not
tied to libc.
However such option is broken at least in the gcc compiler provided in
Alpine Linux, as the system include path (ie: /usr/include) takes
precedence over the gcc private include path:
#include <...> search starts here:
/usr/include
/usr/lib/gcc/x86_64-alpine-linux-musl/10.2.1/include
And the headers in /usr/include are exclusively 64bit.
Since -ffreestanding is currently broken on at least that distro, and
for resilience against future compilers also having the option broken
