Wei Liu c588c002cc tools: remove tmem code and commands

Remove all tmem related code in libxc.

Leave some stubs in libxl in case anyone has linked to those functions
before the removal.

Remove all tmem related commands in xl, all tmem related code in other
utilities we ship.
