Use actual include directory used to install the public header in
COPYING file.
Also, move the input file out of "tools/include/xen/" because that
directory is removed on `make clean`.
We can't used ./configure because $includedir contain another
variable, so the change is done in Makefile.

The notice in the COPYING file in "xen/include/public/COPYING" doesn't
really apply to the files that ultimately are been install at
"/usr/include/xen". The issue are headers in the "sys/" subdirectory
that comes from other projects such as Linux or FreeBSD.
The main issue is that there are two headers that have a different
licence than the MIT licence:
- xen-sys/Linux/gntalloc.h (installed as "sys/gntalloc.h") is public
domain.
- xen-sys/FreeBSD/gntdev.h (installed as "sys/gntdev.h") is BSD-2.
To clarify this, we'll install a COPYING file with a different notice.

the clean use of 'rm -f _*.h' will remove those. _xen_list.h also

cannot be removed as part of the clean rule.
Fix this by dropping the leading underscore, so that the header is not

Today tools/include contains two basically identical header files
generated from the same source. They just differ by the used name space
and they are being generated from different Makefiles via a perl
script.
Prepare to have only one such header by using a more generic namespace
"XEN" for _xen_list.h.
As the original header hasn't been updated in the Xen tree since its
introduction about 10 years ago, and the updates of FreeBSD side have
mostly covered BSD internal debugging aids, just don't generate the
new header during build, especially as using the current FreeBSD
version of the file would require some updates of the perl script,
which are potentially more work than just doing the needed editing by
hand. Additionally this enables to remove the not needed debugging
extensions of FreeBSD.

Running "make uninstall" does not remove all installed files, a
situation which might cause link related issues if xen is re-installed

In order to make uninstall correctly remove the files it is best
the process should be done recursively by mirroring each "install"
target with an "uninstall" who removes the installed files.
An exception to this rule is uninstalling the files produced by
"qemu-xen-dir-remote" and "qemu-xen-traditional-dir", which are external
to the project. These projects do not implement an "uninstall" target so
the files have to be removed manually.

Build can fail if stubdom build is run before tools build because:
1. tools/include build uses relative path and depends on XEN_OS
2. stubdom needs tools/include to be built, at which time XEN_OS is
mini-os and corresponding symlinks are created
3. libraries inside tools needs tools/include to be built, at which
time XEN_OS is the host os name, but symlinks won't be created
because they are already there
4. libraries get the wrong headers and fail to build
Since both tools and stubdom build need the public headers, we build
tools/include before stubdom and tools. Remove runes in stubdom and
tools to avoid building tools/include more than once.
Provide a new dist target for tools/include. Hook up the install,
clean, dist and distclean targets for tools/include.
The new arrangement ensures tools build gets the correct headers
because XEN_OS is set to host os when building tools/include. As for
stubdom, it explicitly links to the mini-os directory without relying
on XEN_OS so it should be fine.

The original rule to target "distclean" in tools/Rules.mk was in effect
"make clean". It should be "make distclean".
However not all Makefiles in subdirectories have distclean target
defined. So this patch also adds a bunch of distclean targets to various
Makefiles. They only depend on clean target and don't have any actions
in most cases.
With the patch applied, following command outputs 0 results:
find tools -name 'Makefile*' -exec grep -L 'distclean' {} \+ \

Ocaml and libfsimage are known to have distclean defined in a dedicated
