This patch is adding a way to enable/disable inguest pagefault
events. It introduces the xc_monitor_inguest_pagefault function
and adds the inguest_pagefault_disabled in the monitor structure.
This is needed by the introspection so it will only get gla
faults and not get spammed with other faults.
In p2m_mem_access_check() we emulate so no event will get sent.

This patch adds the old value param and the onchangeonly option
to the VM_EVENT_REASON_MOV_TO_MSR event.
The param was added to the vm_event_mov_to_msr struct and to the
hvm_monitor_msr function. Finally I've changed the bool_t param
to a bool for the hvm_msr_write_intercept function.

Mixed throughout libxc are uint32_t, int, and domid_t for domid parameters.
With a signed type, and an explicitly 16-bit type, it is exceedingly difficult
to construct an INVALID_DOMID constant which works with all of them. (The
main problem being that domid_t gets unconditionally zero extended when
promoted to int for arithmatic.)
Libxl uses uint32_t consistently everywhere, so alter libxc to match.

If case of a vm_event with the emulate_flags set, if the instruction
is not implemented by the emulator, the monitor should be notified instead
of directly injecting a hw exception.
This behavior can be used to re-execute an instruction not supported by
the emulator using the real processor (e.g. altp2m) instead of just
crashing.

The ARM SMC instructions are already configured to trap to Xen by default. In
this patch we allow a user-space process in a privileged domain to receive
notification of when such event happens through the vm_event subsystem by
introducing the PRIVILEGED_CALL type.
The intended use-case for this feature is for a monitor application to be able
insert tap-points into the domU kernel-code. For this task only unconditional
SMC instruction should be used.

Since in-guest debug exceptions are now unconditionally trapped to Xen, adding
a hook for vm_event subscribers to tap into this new always-on guest event. We
rename along the way hvm_event_breakpoint_type to hvm_event_type to better
match the various events that can be passed with it. We also introduce the
necessary monitor_op domctl's to enable subscribing to the events.
This patch also provides monitor subscribers to int3 events proper access
to the instruction length necessary for accurate event-reinjection. Without
this subscribers manually have to evaluate if the int3 instruction has any
prefix attached which would change the instruction length.

Previously, subscribing to MSR write events was an all-or-none
approach, with special cases for introspection MSR-s. This patch
allows the vm_event consumer to specify exactly what MSR-s it is
interested in, and as a side-effect gets rid of the
vmx_introspection_force_enabled_msrs[] special case.
The patch also introduces arch_monitor_init_domain() and
arch_monitor_cleanup_domain(), to do monitor-specific work
(as opposed to the previous way of doing all the setup in
vm_event_init_domain() / vm_event_cleanup_domain()).
This replaces the previously posted "xen: Filter out MSR write
events" patch.

Added support for a new class of vm_events: VM_EVENT_REASON_REQUEST,
sent via HVMOP_request_vm_event. The guest can request that a
generic vm_event (containing only the vm_event-filled guest registers
as information) be sent to userspace by setting up the correct
registers and doing a VMCALL. For example, for a 32-bit guest, this
means: EAX = 34 (hvmop), EBX = 24 (HVMOP_guest_request_vm_event),
ECX = 0 (NULL required for the hypercall parameter, reserved).

some redundancy and allow for an easier time when adding vm_events
for new control registers in the future, by having a single
VM_EVENT_REASON_WRITE_CTRLREG vm_event type, meant to serve CR0,
CR3, CR4 and (newly introduced) XCR0. The actual control register
will be deduced by the new .index field in vm_event_write_ctrlreg

to Xen to start pulling responses off the corresponding rings. In this patch
we retire these memops and add them to the option to the vm_event_op domctl.
The vm_event_op domctl suboptions are the same for each ring thus we
consolidate them into XEN_VM_EVENT_ENABLE/DISABLE/RESUME.
As part of this patch in libxc we also rename the mem_access_enable/disable
functions to monitor_enable/disable and move them into xc_monitor.c.
