xc_pm_get_pxstat() requires the caller to allocate the pt and trans_pt
buffers but then calls xc_pm_get_max_px() to determine how big they are
(and hence how much Xen will copy into them). This is susceptible to
races if xc_pm_get_max_px() changes so avoid the problem by requiring
the caller to also pass in the size of the buffers.

This may have been necessary in the dim and distant past, but it is broken
today. Linux specifically supports concatenating CPIO fragments of differing
compressions, and any attempt to interpret it with a single algorithm may
corrupt later parts.
This was an unexpected discovery while trying to test Xen's gunzip()

xc_cpuid_apply_policy() provides compatibility for migration of a pre-4.14 VM
where no CPUID data was provided in the stream.
It guesses the various max-leaf limits, based on what was true at the time of
writing, but this was not correctly adapted when speculative security issues
forced the advertisement of new feature bits. Of note are:
* LFENCE-DISPATCH, in leaf 0x80000021.eax
* BHI-CTRL, in leaf 0x7[2].edx
In both cases, a VM booted on a security-patched Xen 4.13, and then migrated
on to any newer version of Xen on the same or compatible hardware would have
these features stripped back because Xen is still editing the cpu-policy for
sanity behind the back of the toolstack.
For VMs using BHI_DIS_S to mitigate Native-BHI, this resulted in a failure to
restore the guests MSR_SPEC_CTRL setting:

When dom0 is PVH, and passthrough a device to dumU, xl will
use the gsi number of device to do a pirq mapping, see
pci_add_dm_done->xc_physdev_map_pirq, but the gsi number is
got from file /sys/bus/pci/devices/<sbdf>/irq, that confuses
irq and gsi, they are in different space and are not equal,
so it will fail when mapping.
To solve this issue, to get the real gsi and add a new function
xc_physdev_map_pirq_gsi to get a free pirq for gsi.
Note: why not use current function xc_physdev_map_pirq, because
it doesn't support to allocate a free pirq, what's more, to
prevent changing it and affecting its callers, so add
xc_physdev_map_pirq_gsi.
Besides, PVH dom0 doesn't have PIRQs flag, it doesn't do
PHYSDEVOP_map_pirq for each gsi. So grant function callstack
pci_add_dm_done->XEN_DOMCTL_irq_permission will fail at function
domain_pirq_to_irq. And old hypercall XEN_DOMCTL_irq_permission
requires passing in pirq, it is not suitable for PVH dom0 that
doesn't have PIRQs to grant irq permission.
To solve this issue, use the another hypercall
XEN_DOMCTL_gsi_permission to grant the permission of irq(
translate from gsi) to dumU when dom0 has no PIRQs.

To change the cd-rom medium, libxl will:
- QMP eject the medium from QEMU
- block-detach the old PV disk
- block-attach the new PV disk
- QMP change the medium to the new PV disk by fdset-id
The QMP code is reused, and remove and attach are implemented here.
The stubdom must internally handle adding /dev/xvdc to the appropriate
fdset. libxl in dom0 doesn't see the result of adding to the fdset as
that is internal to the stubdom, but the fdset's opaque fields will be
set to stub-devid:$devid, so libxl can identify it. $devid is common
between the stubdom and libxl, so it can be identified on both side.
The stubdom will name the device xvdY regardless of the guest name hdY,
sdY, or xvdY, but the stubdom will be assigned the same devid
facilitating lookup. Because the stubdom add-fd call is asynchronous,
libxl needs to poll query-fdsets to identify when add-fd has completed.
For cd-eject, we still need to attach the empty vbd. This is necessary
since xenstore is used to determine that hdc exists. Otherwise after
eject, hdc would be gone and the cd-insert would fail to find the drive
to insert new media.
