Roger Pau Monne f1652ccc90 libxl: add support for parsing MSR features
Introduce support for handling MSR features in
libxl_cpuid_parse_config(). The MSR policies are added to the
libxl_cpuid_policy like the CPUID one, which gets passed to
xc_cpuid_apply_policy().

This allows existing users of libxl to provide MSR related features as
key=value pairs to libxl_cpuid_parse_config() without requiring the
usage of a different API.

Roger Pau Monne 1299ea8d76 libxl: use the cpuid feature names from cpufeatureset.h
The current implementation in libxl_cpuid_parse_config() requires
keeping a list of cpuid feature bits that should be mostly in sync
with the contents of cpufeatureset.h.

Avoid such duplication by using the automatically generated list of
cpuid features in INIT_FEATURE_NAMES in order to map feature names to
featureset bits, and then translate from featureset bits into cpuid
leaf, subleaf, register tuple.

Note that the full contents of the previous cpuid translation table
can't be removed. That's because some feature names allowed by libxl
are not described in the featuresets, or because naming has diverged
and the previous nomenclature is preserved for compatibility reasons.

Should result in no functional change observed by callers, albeit some
new cpuid features will be available as a result of the change.

While there constify cpuid_flags name field.

Roger Pau Monne 53307f5c5d libxl: introduce MSR data in libxl_cpuid_policy
Add a new array field to libxl_cpuid_policy in order to store the MSR
policies.

Adding the MSR data in the libxl_cpuid_policy_list type is done so
that existing users can seamlessly pass MSR features as part of the
CPUID data, without requiring the introduction of a separate
domain_build_info field, and a new set of handlers functions.

Note that support for parsing the old JSON format is kept, as that's
required in order to restore domains or received migrations from
previous tool versions. Differentiation between the old and the new
formats is done based on whether the contents of the 'cpuid' field is
an array or a map JSON object.

Roger Pau Monne 9f6f7e07d0 libxl: change the type of libxl_cpuid_policy_list
Currently libxl_cpuid_policy_list is an opaque type to the users of

Change the type to instead be a structure that contains one array for
CPUID policies, in preparation for it also holding another array for

libxl: fix guest kexec - skip cpuid policy

When a domain performs a kexec (soft reset), libxl__build_pre() is
called with the existing domid. Calling libxl__cpuid_legacy() on the
existing domain fails since the cpuid policy has already been set, and
the guest isn't rebuilt and doesn't kexec.

Roger Pau Monné f90615ce03 x86/p2m: free the paging memory pool preemptively
The paging memory pool is currently freed in two different places:
from {shadow,hap}_teardown() via domain_relinquish_resources() and
from {shadow,hap}_final_teardown() via complete_domain_destroy().
While the former does handle preemption, the later doesn't.

Attempt to move as much p2m related freeing as possible to happen
before the call to {shadow,hap}_teardown(), so that most memory can be
freed in a preemptive way. In order to avoid causing issues to
existing callers leave the root p2m page tables set and free them in
{hap,shadow}_final_teardown(). Also modify {hap,shadow}_free to free
the page immediately if the domain is dying, so that pages don't
accumulate in the pool when {shadow,hap}_final_teardown() get called.

Move altp2m_vcpu_disable_ve() to be done in hap_teardown(), as that's
the place where altp2m_active gets disabled now.

Modify {hap,shadow}_free to free the page immediately if the domain is

{shadow,hap}_final_teardown() get called. This is to limit the amount of
work which needs to be done there (in a non-preemptable manner).

Note the call to shadow_free() in shadow_free_p2m_page() is moved after

Roger Pau Monné b8f4a5de68 x86/shadow: tolerate failure in shadow_prealloc()
Prevent _shadow_prealloc() from calling BUG() when unable to fulfill
the pre-allocation and instead return true/false. Modify
shadow_prealloc() to crash the domain on allocation failure (if the
domain is not already dying), as shadow cannot operate normally after
that. Modify callers to also gracefully handle {_,}shadow_prealloc()
failing to fulfill the request.

Note this in turn requires adjusting the callers of
sh_make_monitor_table() also to handle it returning INVALID_MFN.
sh_update_paging_modes() is also modified to add additional error
paths in case of allocation failure, some of those will return with
null monitor page tables (and the domain likely crashed). This is no
different that current error paths, but the newly introduced ones are
more likely to trigger.

The now added failure points in sh_update_paging_modes() also require
that on some error return paths the previous structures are cleared,
and thus monitor table is null.

While there adjust the 'type' parameter type of shadow_prealloc() to
unsigned int rather than u32.

Roger Pau Monné 97258d8819 livepatch: do not ignore sections with 0 size
A side effect of ignoring such sections is that symbols belonging to
them won't be resolved, and that could make relocations belonging to
other sections that reference those symbols fail.

For example it's likely to have an empty .altinstr_replacement with
symbols pointing to it, and marking the section as ignored will
prevent the symbols from being resolved, which in turn will cause any
relocations against them to fail.

In order to solve this do not ignore sections with 0 size, only ignore
sections that don't have the SHF_ALLOC flag set.

Special case such empty sections in move_payload so they are not taken
into account in order to decide whether a livepatch can be safely

Roger Pau Monné f21d287506 tools/libxl: don't allow IOMMU usage with PoD
Prevent libxl from creating guests that attempts to use PoD together
with an IOMMU, even if no devices are actually assigned.

While the hypervisor could support using PoD together with an IOMMU as
long as no devices are assigned, such usage seems doubtful. There's no
guarantee the guest has PoD no longer be active, and thus a later
assignment of a PCI device to such domain could fail.

Preventing the usage of PoD together with an IOMMU at guest creation
avoids having to add checks for active PoD entries in the device

Roger Pau Monné abfa80967b x86/hap: do not switch on log dirty for VRAM tracking
XEN_DMOP_track_dirty_vram possibly calls into paging_log_dirty_enable

This was detected as a result of the following assert triggering when

Such assert triggered because the stubdom used
XEN_DMOP_track_dirty_vram while dom0 was in the middle of executing
XEN_DOMCTL_SHADOW_OP_OFF, and so log dirty become enabled while
retiring the old structures, thus leading to new entries being
populated in already clear slots.

Fix this by not enabling log dirty for VRAM tracking, similar to what
is done when using shadow instead of HAP. Call
p2m_enable_hardware_log_dirty when enabling VRAM tracking in order to
get some hardware assistance if available. As a side effect the memory
pressure on the p2m pool should go down if only VRAM tracking is
enabled, as the dirty bitmap is no longer allocated.

Note that paging_log_dirty_range (used to get the dirty bitmap for
VRAM tracking) doesn't use the log dirty bitmap, and instead relies on
checking whether each gfn on the range has been switched from
p2m_ram_logdirty to p2m_ram_rw in order to account for dirty pages.

Roger Pau Monné 1a914256dc x86/cpuid: support LFENCE always serialising CPUID bit
AMD Milan (Zen3) CPUs have an LFENCE Always Serialising CPUID bit in
leaf 80000021.eax. Previous AMD versions used to have a user settable
bit in DE_CFG MSR to select whether LFENCE was dispatch serialising,
which Xen always attempts to set. The forcefully always on setting is
due to the addition of SEV-SNP so that a VMM cannot break the
confidentiality of a guest.

In order to support this new CPUID bit move the LFENCE_DISPATCH
synthetic CPUID bit to map the hardware bit (leaving a hole in the
synthetic range) and either rely on the bit already being set by the
native CPUID output, or attempt to fake it in Xen by modifying the
DE_CFG MSR. This requires adding one more entry to the featureset to
support leaf 80000021.eax.

The bit is always exposed to guests by default even if the underlying
hardware doesn't support leaf 80000021. Note that Xen doesn't allow
guests to change the DE_CFG value, so once set by Xen LFENCE will always
be serialising.

Note that the access to DE_CFG by guests is left as-is: reads will
unconditionally return LFENCE_SERIALISE bit set, while writes are
silently dropped.

Roger Pau Monné a51f2df5dc efi: fix alignment of function parameters in compat mode
Currently the max_store_size, remain_store_size and max_size in

In file included from compat.c:30:

Fix this by bouncing the variables on the stack in order for them to

Note this could be done in a more selective manner to only apply to
compat code calls, but given the overhead of making an EFI call doing
an extra copy of 3 variables doesn't seem to warrant the special

A fix for a previous issue removed the pirqs from the domain tree when
they are unbound in order to prevent shared pirqs from triggering a
BUG_ON in __pirq_guest_unbind if they are unbound multiple times. That
caused free_domain_pirqs to no longer unmap the pirqs because they
are gone from the domain pirq tree, thus leaving stale unbound pirqs
after domain destruction if the domain had mapped dpci pirqs after
shutdown.

Take a different approach to fix the original issue, instead of
removing the pirq from d->pirq_tree clear the flags of the dpci pirq
struct to signal that the pirq is now unbound. This prevents calling

Roger Pau Monné d785e076b3 x86/irq: fix infinite loop in irq_move_cleanup_interrupt
If Xen enters irq_move_cleanup_interrupt with a dynamic vector below
IRQ_MOVE_CLEANUP_VECTOR pending in IRR (0x20 or 0x21) that's also
designated for a cleanup it will enter a loop where
irq_move_cleanup_interrupt continuously sends a cleanup IPI (vector
0x22) to itself while waiting for the vector with lower priority to be

Roger Pau Monné d95f45073c x86/msr: fix handling of MSR_IA32_PERF_{STATUS/CTL}
Currently a PV hardware domain can also be given control over the CPU
frequency, and such guest is allowed to write to MSR_IA32_PERF_CTL.
However since commit 322ec7c89f the default behavior has been changed
to reject accesses to not explicitly handled MSRs, preventing PV
guests that manage CPU frequency from reading
MSR_IA32_PERF_{STATUS/CTL}.

Additionally some HVM guests (Windows at least) will attempt to read
MSR_IA32_PERF_CTL and will panic if given back a #GP fault:

  vmx.c:3035:d8v0 RDMSR 0x00000199 unimplemented
  d8v0 VIRIDIAN CRASH: 3b c0000096 fffff806871c1651 ffffda0253683720 0

Move the handling of MSR_IA32_PERF_{STATUS/CTL} to the common MSR
handling shared between HVM and PV guests, and add an explicit case
for reads to MSR_IA32_PERF_{STATUS/CTL}.

Restore previous behavior and allow PV guests with the required
permissions to read the contents of the mentioned MSRs. Non privileged
guests will get 0 when trying to read those registers, as writes to
MSR_IA32_PERF_CTL by such guest will already be silently dropped.

Roger Pau Monné fc8200a6ad x86/vpt: fix race when migrating timers between vCPUs
The current vPT code will migrate the emulated timers between vCPUs
(change the pt->vcpu field) while just holding the destination lock,
either from create_periodic_time or pt_adjust_global_vcpu_target if
the global target is adjusted. Changing the periodic_timer vCPU field
in this way creates a race where a third party could grab the lock in
the unlocked region of pt_adjust_global_vcpu_target (or before
create_periodic_time performs the vcpu change) and then release the
lock from a different vCPU, creating a locking imbalance.

Introduce a per-domain rwlock in order to protect periodic_time
migration between vCPU lists. Taking the lock in read mode prevents
any timer from being migrated to a different vCPU, while taking it in
write mode allows performing migration of timers across vCPUs. The
per-vcpu locks are still used to protect all the other fields from the
periodic_timer struct.

Note that such migration shouldn't happen frequently, and hence
there's no performance drop as a result of such locking.

Roger Pau Monné c23274fd04 x86/ept: flush cache when modifying PTEs and sharing page tables
Modifications made to the page tables by EPT code need to be written
to memory when the page tables are shared with the IOMMU, as Intel
IOMMUs can be non-coherent and thus require changes to be written to
memory in order to be visible to the IOMMU.

In order to achieve this make sure data is written back to memory
after writing an EPT entry when the recalc bit is not set in
atomic_write_ept_entry. If such bit is set, the entry will be
adjusted and atomic_write_ept_entry will be called a second time
without the recalc bit set. Note that when splitting a super page the
new tables resulting of the split should also be written back.

Failure to do so can allow devices behind the IOMMU access to the
stale super page, or cause coherency issues as changes made by the
processor to the page tables are not visible to the IOMMU.

This allows to remove the VT-d specific iommu_pte_flush helper, since
the cache write back is now performed by atomic_write_ept_entry, and
hence iommu_iotlb_flush can be used to flush the IOMMU TLB. The newly
used method (iommu_iotlb_flush) can result in less flushes, since it
might sometimes be called rightly with 0 flags, in which case it
becomes a no-op.

ept_next_level was passing a live PTE pointer to ept_set_middle_entry,
which was then modified without taking into account that the PTE could
be part of a live EPT table. This wasn't a security issue because the
pages returned by p2m_alloc_ptp are zeroed, so adding such an entry
before actually initializing it didn't allow a guest to access
physical memory addresses it wasn't supposed to access.

This is part of XSA-328.

Roger Pau Monné 5b718d24e8 x86/tlb: fix assisted flush usage
Commit e9aca9470e introduced a regression when avoiding sending
IPIs for certain flush operations. Xen page fault handler
(spurious_page_fault) relies on blocking interrupts in order to
prevent handling TLB flush IPIs and thus preventing other CPUs from
removing page tables pages. Switching to assisted flushing avoided such
IPIs, and thus can result in pages belonging to the page tables being
removed (and possibly re-used) while __page_fault_type is being
executed.

Force some of the TLB flushes to use IPIs, thus avoiding the assisted
TLB flush. Those selected flushes are the page type change (when
switching from a page table type to a different one, ie: a page that
has been removed as a page table) and page allocation. This sadly has
a negative performance impact on the pvshim, as less assisted flushes
can be used. Note the flush in grant-table code is also switched to
use an IPI even when not strictly needed. This is done so that a
common arch_flush_tlb_mask can be introduced and always used in common
code.

Introduce a new flag (FLUSH_FORCE_IPI) and helper to force a TLB flush
using an IPI (x86 only). Note that the flag is only meaningfully defined
when the hypervisor supports PV or shadow paging mode, as otherwise
hardware assisted paging domains are in charge of their page tables and
won't share page tables with Xen, thus not influencing the result of
page walks performed by the spurious fault handler.

Just passing this new flag when calling flush_area_mask prevents the
usage of the assisted flush without any other side effects.

Note the flag is not defined on Arm.

Roger Pau Monné 0e2e54966a mm: fix public declaration of struct xen_mem_acquire_resource
XENMEM_acquire_resource and it's related structure is currently inside
a __XEN__ or __XEN_TOOLS__ guarded section to limit it's scope to the
hypervisor or the toolstack only. This is wrong as the hypercall is
already being used by the Linux kernel at least, and as such needs to
be public.

Also switch the usage of uint64_aligned_t to plain uint64_t, as
uint64_aligned_t is only to be used by the toolstack. Doing such
change will reduce the size of the structure on 32bit x86 by 4bytes,
since there will be no padding added after the frame_list handle.

This is fine, as users of the previous layout will allocate 4bytes of
padding that won't be read by Xen, and users of the new layout won't
allocate those, which is also fine since Xen won't try to access them.

Note that the structure already has compat handling, and such handling
will take care of copying the right size (ie: minus the padding) when
called from a 32bit x86 context. This is true for the compat code both
before and after this patch, since the structures in the memory.h
compat header are subject to a pragma pack(4), which already removed
the trailing padding that would otherwise be introduced by the
alignment of the frame field to 8 bytes.

Mediated access to the RTC was provided for PVHv1 dom0 using the PV
code paths (guest_io_{write/read}), but those accesses where never
implemented for PVHv2 dom0. This patch provides such mediated accesses
to the RTC for PVH dom0, just like it's provided for a classic PV
dom0.

Pull out some of the RTC logic from guest_io_{read/write} into
specific helpers that can be used by both PV and HVM guests. The
setup of the handlers for PVH is done in rtc_init, which is already
used to initialize the fully emulated RTC.

Without this a Linux PVH dom0 will read garbage when trying to access
the RTC, and one vCPU will be constantly looping in
rtc_timer_do_work.

Note that such issue doesn't happen on domUs because the ACPI
NO_CMOS_RTC flag is set in FADT, which prevents the OS from accessing
the RTC. Also the X86_EMU_RTC flag is not set for PVH dom0, as the
accesses are not emulated but rather forwarded to the physical
hardware.

PVHv1 dom0 was given access to all PHYSDEVOP hypercalls, and such

the switch in hvm_physdev_op was more complicated than required, and
relied on PVHv2 dom0 not having PIRQ support in order to prevent
access to some PV specific PHYSDEVOPs.

Fix this by moving the default case to the bottom of the switch, since
there's no need for any fall through now. Also remove the hardware
domain check, as all the not explicitly listed PHYSDEVOPs are
forbidden for HVM domains.

Finally tighten the condition to allow usage of
PHYSDEVOP_pci_mmcfg_reserved: apart from having vPCI enabled it should
only be used by the hardware domain. Note that the code in
do_physdev_op is already restricting the call to privileged domains
only, but it can be further restricted to the hardware domain only, as
other privileged domains don't have access to MMCFG regions anyway.

Roger Pau Monné 17b997aa1e x86/hap: be more selective with assisted TLB flush
When doing an assisted flush on HAP the purpose of the
on_selected_cpus is just to trigger a vmexit on remote CPUs that are
in guest context, and hence just using is_vcpu_dirty_cpu is too lax,
also check that the vCPU is running. Due to the lazy context switching
done by Xen dirty_cpu won't always be cleared when the guest vCPU is
not running, and hence relying on is_running allows more fine grained
control of whether the vCPU is actually running.

I've measured the time of the non-local branch of flush_area_mask

Use Xen's L0 HVMOP_flush_tlbs hypercall in order to perform flushes.
This greatly increases the performance of TLB flushes when running

when running in shim mode.

The following figures are from a PV guest running `make -j32 xen` in
shim mode with 32 vCPUs and HAP.

Using x2APIC and ALLBUT shorthand:
real	4m35.973s
user	4m35.110s
sys	36m24.117s

Using L0 assisted flush:
real 1m2.596s
user 4m34.818s
sys 5m16.374s

The implementation adds a new hook to hypervisor_ops so other
enlightenments can also implement such assisted flush just by filling
the hook.

Note that the Xen implementation completely ignores the dirty CPU mask
and the linear address passed in, and always performs a global TLB
flush on all vCPUs. This is a limitation of the hypercall provided by
Xen. Also note that local TLB flushes are not performed using the
assisted TLB flush, only remote ones.

Roger Pau Monné c2b4e23fdd x86/tlb: allow disabling the TLB clock
The TLB clock is helpful when running Xen on bare metal because when
doing a TLB flush each CPU is IPI'ed and can keep a timestamp of the
last flush.

This is not the case however when Xen is running virtualized, and the
underlying hypervisor provides mechanism to assist in performing TLB
flushes: Xen itself for example offers a HVMOP_flush_tlbs hypercall in
order to perform a TLB flush without having to IPI each CPU. When
using such mechanisms it's no longer possible to keep a timestamp of
the flushes on each CPU, as they are performed by the underlying
hypervisor.

Offer a boolean in order to signal Xen that the timestamped TLB
shouldn't be used. This avoids keeping the timestamps of the flushes,
and also forces NEED_FLUSH to always return true.

No functional change intended, as this change doesn't introduce any
user that disables the timestamped TLB.

Introduce a specific flag to request a HVM guest linear TLB flush,
which is an ASID/VPID tickle that forces a guest linear to guest
physical TLB flush for all HVM guests.

This was previously unconditionally done in each pre_flush call, but
that's not required: HVM guests not using shadow don't require linear
TLB flushes as Xen doesn't modify the pages tables the guest runs on
in that case (ie: when using HAP). Note that shadow paging code
already takes care of issuing the necessary flushes when the shadow
page tables are modified.

In order to keep the previous behavior modify all shadow code TLB
flushes to also flush the guest linear to physical TLB if the guest is
HVM. I haven't looked at each specific shadow code TLB flush in order
to figure out whether it actually requires a guest TLB flush or not,
so there might be room for improvement in that regard.

Also perform ASID/VPID flushes when modifying the p2m tables as it's a
requirement for AMD hardware. Finally keep the flush in
switch_cr3_cr4, as it's not clear whether code could rely on
switch_cr3_cr4 also performing a guest linear TLB flush. A following

Roger Pau Monne 25df013e7a x86/nvmx: split updating RVI from SVI in nvmx_update_apicv
Updating SVI is required when an interrupt has been injected using the
Ack on exit VMEXIT feature, so that the in service interrupt in the
GUEST_INTR_STATUS matches the vector that is signaled in
VM_EXIT_INTR_INFO.

Updating RVI however is not tied to the Ack on exit feature, as it
signals the next vector to be injected, and hence should always be
updated to the next pending vector, regardless of whether Ack on exit
is enabled.

When not using the Ack on exit feature preserve the previous vector in
SVI, so that it's not lost when RVI is updated to contain the pending
vector to inject.

Roger Pau Monné c9495bd7df x86/hap: improve hypervisor assisted guest TLB flush
The current implementation of the hypervisor assisted flush for HAP is
extremely inefficient.

First of all there's no need to call paging_update_cr3, as the only
relevant part of that function when doing a flush is the ASID vCPU
flush, so just call that function directly.

Since hvm_asid_flush_vcpu is protected against concurrent callers by
using atomic operations there's no need anymore to pause the affected
vCPUs.

Finally the global TLB flush performed by flush_tlb_mask is also not
necessary, since we only want to flush the guest TLB state it's enough
to trigger a vmexit on the pCPUs currently holding any vCPU state, as
such vmexit will already perform an ASID/VPID update, and thus clear
the guest TLB.

Roger Pau Monné a798bac54f x86/hvm: allow ASID flush when v != current
Current implementation of hvm_asid_flush_vcpu is not safe to use
unless the target vCPU is either paused or the currently running one,
as it modifies the generation without any locking.

Fix this by using atomic operations when accessing the generation
field, both in hvm_asid_flush_vcpu_asid and other ASID functions. This
allows to safely flush the current ASID generation. Note that for the
flush to take effect if the vCPU is currently running a vmexit is
required.

Compilers will normally do such writes and reads as a single
instruction, so the usage of atomic operations is mostly used as a
safety measure.

Note the same could be achieved by introducing an extra field to
hvm_vcpu_asid that signals hvm_asid_handle_vmenter the need to call
hvm_asid_flush_vcpu on the given vCPU before vmentry, this however
seems unnecessary as hvm_asid_flush_vcpu itself only sets two vCPU
fields to 0, so there's no need to delay this to the vmentry ASID
helper.

This is not a bugfix as no callers that would violate the assumptions
listed in the first paragraph have been found, but a preparatory
change in order to allow remote flushing of HVM vCPUs.

Add helpers to track when running in NMI handler context. This is

The SDM states that no NMI can be delivered while handling a NMI
until the processor has executed an iret instruction. It's possible
however that another fault is received while handling the NMI (a #MC
for example), and thus the iret from that fault would allow further
NMIs to be injected while still processing the previous one, and
hence an integer is needed in order to keep track of in service NMIs.
The added macros only track when the execution context is in the NMI

Don't allow cpu_hotplug_begin to fail by converting the trylock into a

The exclusive lock usage in register_cpu_notifier is also converted
into a blocking lock acquisition, as it was previously not allowed to

This is meaningful when running Xen in shim mode, since VCPU_{up/down}

Allow a CPU already holding the lock in write mode to also lock it in

In order to do this reserve 12bits of the lock, this allows to support
up to 4096 CPUs. Also reduce the write lock mask to 2 bits: one to
signal there are pending writers waiting on the lock and the other to
signal the lock is owned in write mode.

This reduces the maximum number of concurrent readers from 16777216 to

Convert the lock into a rw lock, and take the lock in read mode in

Roger Pau Monné c47984aabe nvmx: implement support for MSR bitmaps
Current implementation of nested VMX has a half baked handling of MSR
bitmaps for the L1 VMM: it maps the L1 VMM provided MSR bitmap, but
doesn't actually load it into the nested vmcs, and thus the nested
guest vmcs ends up using the same MSR bitmap as the L1 VMM.

This is wrong as there's no assurance that the set of features enabled
for the L1 vmcs are the same that L1 itself is going to use in the
nested vmcs, and thus can lead to misconfigurations.

For example L1 vmcs can use x2APIC virtualization and virtual
interrupt delivery, and thus some x2APIC MSRs won't be trapped so that
they can be handled directly by the hardware using virtualization
extensions. On the other hand, the nested vmcs created by L1 VMM might
not use any of such features, so using a MSR bitmap that doesn't trap
accesses to the x2APIC MSRs will be leaking them to the underlying
hardware.

Fix this by crafting a merged MSR bitmap between the one used by L1
and the nested guest.
