This patch adds basic framework for TEE mediators. Guests can't talk
to TEE directly, we need some entity that will intercept request
and decide what to do with them. "TEE mediator" is a such entity.
This is how it works: user can build XEN with multiple TEE mediators
(see the next patches, where OP-TEE mediator is introduced).
TEE mediator register self with REGISTER_TEE_MEDIATOR() macro in the
same way, as device drivers use DT_DEVICE_START()/DT_DEVICE_END()
macros.
At run-time, during initialization, framework calls probe() function
for each available mediator driver to find which TEE is installed
on the platform. Then generic vSMC handler will call selected mediator
when it intercept SMC/HVC that belongs to TEE OS or TEE application.

There is now enough complexity that a fuzzing harness is a good idea, and
enough supporting logic to implement one which AFL seems happy with.
Take the existing recalculate_synth() helper and export it as
x86_cpuid_policy_recalc_synth(), as it is needed by the fuzzing harness.
While editing the MAINTAINERS file, insert a related entry which was
accidentally missed from c/s 919ddc3c0 "tools/cpu-policy: Add unit tests", and
sort the lines.

Blktap2 is effectively dead for a few years.
Notable changes in this patch:
0. Unhook blktap2 from build system
1. libxl no longer supports TAP disk backend, with appropriate assertions
added and some code paths now return ERROR_FAIL
2. Tap is no longer a supported backend
3. Remove blktap2 entry from MAINTAINERS
A patch to remove blktap2 directory will come later.

This patch removes all tmem related code and CONFIG_TMEM from the
hypervisor. Also remove tmem hypercalls from the default XSM policy.
It is written as if tmem is disabled and tmem freeable pages is 0.
We will need to keep public/tmem.h around forever to avoid breaking
guests. Remove the hypervisor only part and put guest visible part
under a xen version check. Take the chance to remove trailing
whitespaces.

Subsequent patches will introduce support for more viridian enlightenments
which will make a single source module quite lengthy.
This patch therefore creates a new arch/x86/hvm/viridian sub-directory and
moves viridian.c into that.
The patch also fixes some bad whitespace whilst moving the code and
adjusts the MAINTAINERS file.

Commit message stolen from a reply from Ian:
The git protocol is not just unencrypted, but also unauthenticated.
In theory it is possible to verify the signed tags for actual
releases, but that is a cumbersome process which I very much doubt
anyone really does.
As for the various branch tips, there is currently no way (unless you
have a shell account on xenbits) to get any kind of authenticated
value.
Conversely, if you use an https url, you get some cryptographic
authentication of what you are cloning. The crypto there is far from
perfect but it is massively better than nothing.
Additionally, in general, using and supporting https also means that
*what users are accessing* is encrypted. This enhances user privacy.
In the specific case of the git trees on xenbits this is a very minor
consideration.

oxenstored is our default implementation of xenstore, for platforms
that have ocaml support. We need it to be maintained. Dave Scott,
the only existing maintainer, has had limited availability.
Christian has been reveiwing patches and offering opinions where
necessary, although activity in this area has been quiet and there has
not been a great deal of new development.
Christian's contributions have been sensible and I think it would be a
good idea now to formally make him a maintainer.

This patch relocates mem_access components that are currently mixed with p2m
code into separate files. This better aligns the code with similar subsystems,
such as mem_sharing and mem_paging, which are already in separate files. There
are no code-changes introduced, the patch is mechanical code movement.
On ARM we also relocate the static inline gfn_next_boundary function to p2m.h
as it is a function the mem_access code needs access to.
