Current code calls apic_x2apic_probe which does some initialization
and setup before having enabled x2APIC mode (if it's not already

This can lead to issues if the APIC ID doesn't match the x2APIC ID, as
apic_x2apic_probe calls init_apic_ldr_x2apic_cluster which depending
on the APIC mode might set cpu_2_logical_apicid using the APIC ID
instead of the x2APIC ID (because x2APIC might not be enabled yet).
Fix this by enabling x2APIC before calling apic_x2apic_probe.
As a remark, this was discovered while I was trying to figure out why
one of my test boxes didn't report any iommu faults. The root cause
was that the iommu MSI address field was set using the stale value in
cpu_2_logical_apicid, and thus the iommu fault interrupt would get
lost. Even if the MSI address field gets sets to a correct value
afterwards as soon as a single iommu fault is pending no further
interrupts would get injected, so losing a single iommu fault
interrupt is fatal.

paging_log_dirty_op function takes mm locks from a subject domain and
then attempts to perform copy to operations against the caller domain
in order to copy the result of the hypercall into the caller provided
buffer.
This works fine when the caller is a non-paging domain, but triggers a
lock order panic when the caller is a paging domain due to the fact
that at the point where the copy to operation is performed the subject
domain paging lock is locked, and the copy operation requires
locking the caller p2m lock which has a lower level.
Fix this limitation by adding a bias to the level of control domain mm
locks, so that the lower control domain mm lock always has a level
greater than the higher unprivileged domain lock level. This allows
locking the subject domain mm locks and then locking the control
domain mm locks, while keeping the same lock ordering and the changes
mostly confined to mm-locks.h.
Note that so far only this flow (locking a subject domain locks and
then the control domain ones) has been identified, but not all
possible code paths have been inspected. Hence this solution attempts
to be a non-intrusive fix for the problem at hand, without discarding
further changes in the future if other valid code paths are found that
require more complex lock level ordering.

Commit 042678762 ("x86/iommu: add map-reserved dom0-iommu option to

regions into the PVH Dom0 p2m, and some of those reserved regions
might contain MSIX MMIO regions, hence the need to make sure there are
no mappings established.

Current logic to handle long running operations is flawed because it
doesn't prevent the guest vcpu from running. Fix this by raising a
scheduler softirq when preemption is required, so that the do_softirq
call in the guest entry path performs a rescheduling. Also move the
call to vpci_process_pending into handle_hvm_io_completion, together
with the IOREQ code that handles pending IO instructions.
Note that a scheduler softirq is also raised when the long running
operation is queued in order to prevent the guest vcpu from resuming
execution.

Note that when only mapping/unmapping the ROM BAR a fabricated command
register value is passed to modify_bars which is only used to signal
whether the action is a mapping or unmapping, but the value is never

Flush checking for AMD IOMMU didn't check whether the previous entry
was present, or whether the flags (writable/readable) changed in order
to decide whether a flush should be executed.
Fix this by taking the writable/readable/next-level fields into account,
together with the present bit.
Along these lines the flushing in amd_iommu_map_page() must not be
omitted for PV domains. The comment there was simply wrong: Mappings may
very well change, both their addresses and their permissions. Ultimately
this should honor iommu_dont_flush_iotlb, but to achieve this
amd_iommu_ops first needs to gain an .iotlb_flush hook.
Also make clear_iommu_pte_present() static, to demonstrate there's no
caller omitting the (subsequent) flush.
This is part of XSA-275.

resulting multiboot module list is then handed to the generic boot
process using the physical address in mbi->mods_addr.
This works fine as long as the Xen image doesn't relocate itself, if
there's such a relocation the physical addresses of multiboot module

Fix this by handing the virtual address of the module list to the

AMD has code in amd_iommu_hwdom_init to setup inclusive mappings up to
max_pdx, remove this since it's now a duplication of
arch_iommu_hwdom_init. Note that AMD mapped every page with a valid

below 4GB, so this is a functional change for AMD.

Several people have reported hardware issues (malfunctioning USB
controllers) due to iommu page faults on Intel hardware. Those faults
are caused by missing RMRR (VTd) entries in the ACPI tables. Those can
be worked around on VTd hardware by manually adding RMRR entries on
the command line, this is however limited to Intel hardware and quite
cumbersome to do.
In order to solve those issues add a new dom0-iommu=map-reserved
option that identity maps all regions marked as reserved in the memory
map. Note that regions used by devices emulated by Xen (LAPIC, IO-APIC
or PCIe MCFG regions) are specifically avoided. Note that this option
is available to all Dom0 modes (as opposed to the inclusive option
which only works for PV Dom0).
