Roger Pau Monne 7b3c5b70a3 tools/libxc: disable x2APIC when using nested virtualization
There are issues as reported by osstest when Xen is running nested on
itself and the L1 Xen is using x2APIC. While those are being
investigated, disable announcing the x2APIC feature in CPUID when nested
HVM mode is enabled.

Roger Pau Monne da9290639e x86/vvmx: virtualize x2APIC mode and APIC accesses can't both be enabled
According to the Intel SDM, "virtualize x2APIC mode" and "virtualize
APIC accesses" can't be enabled at the same time, or else a
vm{launch/entry} failure will happen. This was seen when running Xen
nested and with x2APIC enabled:

  (XEN) d3v0 VMLAUNCH error: 0x7
  [...]
  (XEN) *** Control State ***
  (XEN) PinBased=0000003f CPUBased=b6a075fe SecondaryExec=000014fb
  [...]

Fix this by making sure nvmx_update_secondary_exec_control clears the
incompatible bits from the host vmcs before merging it with the nested
vmcs.

This fixes a regression reported by osstest in the
test-amd64-amd64-qemuu-nested-intel job.

Roger Pau Monné b1710040ca x86/apic: allow enabling x2APIC mode regardless of interrupt remapping
x2APIC mode doesn't mandate interrupt remapping, and hence can be
enabled independently. This patch enables x2APIC when available,
regardless of whether there's interrupt remapping support.

This is beneficial specially when running on virtualized environments,
since it reduces the amount of vmexits. For example when sending an
IPI in xAPIC mode Xen performs at least 3 different accesses to the
APIC MMIO region, while when using x2APIC mode a single wrmsr is used.

The following numbers are from a lock profiling of a Xen PV shim
running a Linux PV kernel with 32 vCPUs and xAPIC mode:

(XEN) Global lock flush_lock: addr=ffff82d0804af1c0, lockval=03190319, not locked
(XEN) lock:656153(892606463454), block:602183(9495067321843)

Average lock time: 1360363ns
Average block time: 15767743ns

While the following are from the same configuration but with the shim
using x2APIC mode:

(XEN) Global lock flush_lock: addr=ffff82d0804b01c0, lockval=1adb1adb, not locked
(XEN) lock:1841883(1375128998543), block:1658716(10193054890781)

Average lock time: 746588ns
Average block time: 6145147ns

Enabling x2APIC has halved the average lock time, thus reducing
contention.

Roger Pau Monné c08cbf7fb8 x86/apic: force phys mode if interrupt remapping is disabled
Cluster mode can only be used with interrupt remapping support, since
the top 16bits of the APIC ID are filled with the cluster ID, and
hence on systems where the physical ID is still smaller than 255 the
cluster ID is not. Force x2APIC to use physical mode if there's no
interrupt remapping support.

Note that this requires a further patch in order to enable x2APIC

Roger Pau Monné 9356f9de41 x86/ioapic: only use dest32 with x2apic and interrupt remapping enabled
The IO-APIC code assumes that x2apic being enabled also implies
interrupt remapping being enabled, and hence will use the 32bit
destination field in the IO-APIC entry.

This is safe now, but there's no reason to not enable x2APIC even
without interrupt remapping, and hence the IO-APIC code needs to use
the 32 bit destination field only when both interrupt remapping and
x2APIC are enabled.

Roger Pau Monné 5de961d9c0 x86: do not enable global pages when virtualized on AMD or Hygon hardware
When using global pages a full tlb flush can only be performed by
toggling the PGE bit in CR4, which is usually quite expensive in terms
of performance when running virtualized. This is specially relevant on
AMD or Hygon hardware, which doesn't have the ability to do selective
CR4 trapping, but can also be relevant on e.g. Intel if the underlying
hypervisor also traps accesses to the PGE CR4 bit.

In order to avoid this performance penalty, do not use global pages
when running virtualized on AMD or Hygon hardware. A command line option
'global-pages' is provided in order to allow the user to select whether
global pages will be enabled for PV guests.

The above figures are from a PV shim running on AMD hardware with
32 vCPUs:

PGE enabled, x2APIC mode:

(XEN) Global lock flush_lock: addr=ffff82d0804b01c0, lockval=1adb1adb, not locked
(XEN) lock:1841883(1375128998543), block:1658716(10193054890781)

Average lock time: 746588ns
Average block time: 6145147ns

PGE disabled, x2APIC mode:

(XEN) Global lock flush_lock: addr=ffff82d0804af1c0, lockval=a8bfa8bf, not locked
(XEN) lock:2730175(657505389886), block:2039716(2963768247738)

Average lock time: 240829ns
Average block time: 1453029ns

As seen from the above figures the lock and block time of the flush
lock is reduced to approximately 1/3 of the original value.

Note that XEN_MINIMAL_CR4 and mmu_cr4_features are not modified, and
thus global pages are left enabled for the hypervisor. This is not an
issue because the code to switch the control registers (cr3 and cr4)
already takes into account such situation and performs the necessary
flushes. The same already happens when using XPTI or PCIDE, as the
guest cr4 doesn't have global pages enabled in that case either.

Also note that the suspend and resume code is correct in writing
mmu_cr4_features into cr4 on resume, since that's the cr4 used by the
idle vCPU which is the context used by the suspend and resume routine.

Roger Pau Monné b5087a31ef x86/tlbflush: do not toggle the PGE CR4 bit unless necessary
When PCID is not available Xen does a full tlbflush by toggling the
PGE bit in CR4. This is not necessary if PGE is not enabled, since a
flush can be performed by writing to CR3 in that case.

Change the code in do_tlb_flush to only toggle the PGE bit in CR4 if
it's already enabled, otherwise do the tlb flush by writing to CR3.
This is relevant when running virtualized, since hypervisors don't
usually trap accesses to CR3 when using hardware assisted paging, but
do trap accesses to CR4 specially on AMD hardware, which makes such
accesses much more expensive.

When using posted interrupts on Intel hardware it's possible that the

Fix this by making sure PIR is always synced to IRR in

clear_IO_APIC_pin can be called after the iommu has been enabled, and
using raw reads and writes to modify IO-APIC entries that have been
setup to use interrupt remapping can lead to issues as some of the
fields have different meaning when the IO-APIC entry is setup to point
to an interrupt remapping table entry.

The following ASSERT in AMD IOMMU code triggers afterwards as a result

On Intel hardware there's currently no translation of already enabled

introduce a logic similar to the one used in x2apic_bsp_setup in order

after interrupt remapping has been enabled.

With this change the AMD specific logic to deal with enabled pins

handling of IO-APIC when enabling interrupt remapping regardless of

Current physcaps in XEN_SYSCTL_physinfo are only used by x86, albeit
the capabilities themselves are not x86 specific.

This patch adds support for also reporting the current capabilities on
Arm hardware. Note that on Arm PHYSCAP_hvm is always reported, and
setting PHYSCAP_directio has been moved to common code since the same
logic to set it is used by x86 and Arm.

Roger Pau Monné 021eb25a29 x86/xstate: make use_xsave non-init
LLVM code generation can attempt to load from a variable in the next
condition of an expression under certain circumstances, thus
attempting to load use_xsave regardless of the value of the bsp
variable, which leads to a page fault when the init section has
already been unmapped.

Fix this by making use_xsave non-init, thus preventing the page fault;
use __read_mostly instead. The LLVM bug with the discussion about this
issue can be found at:

Roger Pau Monné ee91a49a44 partially revert "x86/mm: Clean IOMMU flags from p2m-pt code"
This partially reverts commit
854a49a748 by re-adding the logic that
propagates changes to the domain physmap done by p2m_pt_set_entry into
the iommu page tables. Without this logic changes to the guest physmap
are not propagated to the iommu, leaving stale iommu entries that can
leak data, or failing to add new entries.

Note that this commit doesn't re-introduce iommu flags to the cpu page
table entries, since the logic to add/remove entries to the iommu page
tables is based on the p2m type and the mfn.

Roger Pau Monné 260940578d x86/apic: enable x2APIC mode before doing any setup
Current code calls apic_x2apic_probe which does some initialization
and setup before having enabled x2APIC mode (if it's not already
enabled by the firmware).

This can lead to issues if the APIC ID doesn't match the x2APIC ID, as
apic_x2apic_probe calls init_apic_ldr_x2apic_cluster which depending
on the APIC mode might set cpu_2_logical_apicid using the APIC ID
instead of the x2APIC ID (because x2APIC might not be enabled yet).

Fix this by enabling x2APIC before calling apic_x2apic_probe.

As a remark, this was discovered while I was trying to figure out why
one of my test boxes didn't report any iommu faults. The root cause
was that the iommu MSI address field was set using the stale value in
cpu_2_logical_apicid, and thus the iommu fault interrupt would get
lost. Even if the MSI address field gets sets to a correct value
afterwards as soon as a single iommu fault is pending no further
interrupts would get injected, so losing a single iommu fault
interrupt is fatal.

Roger Pau Monné 6fa1755644 amd/npt/shadow: replace assert that prevents creating 2M/1G MMIO entries
The assert was originally added to make sure that higher order
regions (> PAGE_ORDER_4K) could not be used to bypass the
mmio_ro_ranges check performed by p2m_type_to_flags.

This however is already checked in set_mmio_p2m_entry, which makes
sure that higher order mappings don't overlap with mmio_ro_ranges,
thus allowing the creation of high order MMIO mappings safely.

Replace the assert to allow 2M/1G entries to be created for MMIO
regions and add some extra asserts as a replacement to make sure
there's no overlapping with MMIO read-only ranges.

Note that 1G MMIO entries will not be created unless mmio_order is

Roger Pau Monne e5268a3ce3 x86/mm-locks: apply a bias to lock levels for control domain
paging_log_dirty_op function takes mm locks from a subject domain and
then attempts to perform copy to operations against the caller domain
in order to copy the result of the hypercall into the caller provided
buffer.

This works fine when the caller is a non-paging domain, but triggers a
lock order panic when the caller is a paging domain due to the fact
that at the point where the copy to operation is performed the subject
domain paging lock is locked, and the copy operation requires
locking the caller p2m lock which has a lower level.

Fix this limitation by adding a bias to the level of control domain mm
locks, so that the lower control domain mm lock always has a level
greater than the higher unprivileged domain lock level. This allows
locking the subject domain mm locks and then locking the control
domain mm locks, while keeping the same lock ordering and the changes
mostly confined to mm-locks.h.

Note that so far only this flow (locking a subject domain locks and
then the control domain ones) has been identified, but not all
possible code paths have been inspected. Hence this solution attempts
to be a non-intrusive fix for the problem at hand, without discarding
further changes in the future if other valid code paths are found that
require more complex lock level ordering.

Roger Pau Monné 677053fac1 vpci/msix: carve p2m hole for MSIX MMIO regions
Make sure the MSIX MMIO regions don't have p2m entries setup, so that
accesses to them trap into the hypervisor and can be handled by vpci.

Commit 042678762 ("x86/iommu: add map-reserved dom0-iommu option to
map reserved memory ranges") added mappings for all the reserved
regions into the PVH Dom0 p2m, and some of those reserved regions
might contain MSIX MMIO regions, hence the need to make sure there are
no mappings established.

Roger Pau Monné 3e568fa9e1 vpci: fix deferral of long operations
Current logic to handle long running operations is flawed because it
doesn't prevent the guest vcpu from running. Fix this by raising a
scheduler softirq when preemption is required, so that the do_softirq
call in the guest entry path performs a rescheduling. Also move the
call to vpci_process_pending into handle_hvm_io_completion, together
with the IOREQ code that handles pending IO instructions.

Note that a scheduler softirq is also raised when the long running
operation is queued in order to prevent the guest vcpu from resuming
execution.

Roger Pau Monné 0290d553e0 vpci: fix updating the command register
When switching the memory decoding bit in the command register the
rest of the changes where dropped, leading to only the memory decoding
bit being updated.

Fix this by writing the command register once the guest physmap
manipulations are done if there are changes to the memory decoding
bit.

Note that when only mapping/unmapping the ROM BAR a fabricated command
register value is passed to modify_bars which is only used to signal
whether the action is a mapping or unmapping, but the value is never
written to the device command register. Turn the maodify_decoding
ASSERT into an ASSERT_UNREACHABLE and make sure that non-debug builds
won't end up writing to the command register if only modifying the ROM
BAR.

Roger Pau Monné 1a7ffe466c amd/iommu: fix flush checks
Flush checking for AMD IOMMU didn't check whether the previous entry
was present, or whether the flags (writable/readable) changed in order
to decide whether a flush should be executed.

Fix this by taking the writable/readable/next-level fields into account,
together with the present bit.

Along these lines the flushing in amd_iommu_map_page() must not be
omitted for PV domains. The comment there was simply wrong: Mappings may
very well change, both their addresses and their permissions. Ultimately
this should honor iommu_dont_flush_iotlb, but to achieve this
amd_iommu_ops first needs to gain an .iotlb_flush hook.

Also make clear_iommu_pte_present() static, to demonstrate there's no
caller omitting the (subsequent) flush.

This is part of XSA-275.

As I've touched quite a lot of this code in order to add PVH and PV
shim support I would like to keep an eye on incoming changes, and
since I'm also attempting to review patches in this area it's going to
