Julien Grall a56d598e13 Only compile the hypervisor with -Wdeclaration-after-statement
Right now, all tools and hypervisor will be complied with the option
-Wdeclaration-after-statement. While most of the code in the hypervisor
is controlled by us, for tools we may import external libraries.

The build will fail if one of them are using the construct we are
trying to prevent. This is the case when building against Python 3.12
and Yocto:

Looking at the tools directory, a fair few directory already add
-Wno-declaration-after-statement to inhibit the default behavior.

We have always build the hypervisor with the flag, so for now remove
only the flag for anything but the hypervisor. We can decide at later
time whether we want to relax.

Also remove the -Wno-declaration-after-statement in some subdirectory
as the flag is now unnecessary.

Part of the commit message was take from Alexander's first proposal:

Julien Grall 5415b2b211 docs/arm: Document where Xen should be loaded in memory
In commit 9d267c049d ("xen/arm64: Rework the memory layout"),
we decided to require Xen to be loaded below 2 TiB to simplify
the logic to enable the MMU. The limit was decided based on
how known platform boot plus some slack.

We had a recent report that this is not sufficient on the AVA
platform with a old firmware [1]. But the restriction is not
going to change in Xen 4.18. So document the limit clearly
in docs/misc/arm/booting.txt.

Julien Grall 6432228fb5 xen/arm: vtimer: Don't read/use the secure physical timer interrupt for ACPI
Per ACPI 6.5 section 5.2.25 ("Generic Timer Description Table (GTDT)"),
the fields "Secure EL1 Timer GSIV/Flags" are optional and an OS running
in non-secure world is meant to ignore the values.

However, Xen is trying to reserve the value. The ACPI tables for Graviton
2 metal instances will provide the value 0 which is not a correct PPI
(PPIs start at 16) and would have in fact been already reserved by Xen
as this is an SGI. Xen will hit the BUG() and panic().

For the Device-Tree case, I couldn't find a statement suggesting
that the secure physical timer interrupt is ignored. In fact, I have
found some code in Linux using it as a fallback. That said, it should
never be used.

As I am not aware of any issue when booting using Device-Tree, the
physical timer interrupt is only ignored for ACPI.

Julien Grall 39113a8a23 tools/light: Revoke permissions when a PCI detach for HVM domain
Currently, libxl will grant IOMEM, I/O port and IRQ permissions when
a PCI is attached (see pci_add_dm_done()) for all domain types. However,
the permissions are only revoked for non-HVM domain (see do_pci_remove()).

This means that HVM domains will be left with extra permissions. While
this look bad on the paper, the IRQ permissions should be revoked
when the Device Model call xc_physdev_unmap_pirq() and such domain
cannot directly mapped I/O port and IOMEM regions. Instead, this has to
be done by a Device Model.

The Device Model can only run in dom0 or PV stubdomain (upstream libxl
doesn't have support for HVM/PVH stubdomain).

For PV/PVH stubdomain, the permission are properly revoked, so there is
no security concern.

This leaves dom0. There are two cases:
  1) Privileged: Anyone gaining access to the Device Model would already
have large control on the host.
  2) Deprivileged: PCI passthrough require PHYSDEV operations which
are not accessible when the Device Model is restricted.

So overall, it is believed that the extra permissions cannot be exploited.

Rework the code so the permissions are all removed for HVM domains.
This needs to happen after the QEMU has detached the device. So
the revocation is now moved to pci_remove_detached().

Also add a comment on top of the error message when the PIRQ cannot
be unbind to explain this could be a spurious error as QEMU may have
already done it.

The implementation of write_pte() is pretty much the same on arm32
and arm64. So it would be good to consolidate it as this would help
to clarify the requirements when using the helper.

Take the opportunity to switch from assembly to call macros. Note there
is a difference on arm32 because the option was not specified. But this
meant 'sy' (system wide).

Futhermore, the requirements for the ISB is incomplete. Per the Arm Arm,
(Armv7 DDI406C.d A3.8.3 and Armv8 DDI 0487J.a B2.3.12), DSB will only
affect explicit accesses. So an ISB is necessary after DSB to ensure
the completion. Having an ISB after each update to the page-tables is
probably too much, so let the caller add the instruction when it is
convenient.

Lastly, the barrier in write_pte() may be too restrictive but I haven't

At the moment, we are mapping the size of the reserved area for Xen
(i.e. 2MB) even if the binary is smaller. We don't exactly know what's
after Xen, so it is not a good idea to map more than necessary for a
couple of reasons:
    * We would need to use break-before-make if the extra PTE needs to
be updated to point to another region
    * The extra area mapped may be mapped again by Xen with different

Julien Grall 804145a161 xen/arm64: head: Don't map too much in boot_third
At the moment, we are mapping the size of the reserved area for Xen
(i.e. 2MB) even if the binary is smaller. We don't exactly know what's
after Xen, so it is not a good idea to map more than necessary for a
couple of reasons:
    * We would need to use break-before-make if the extra PTE needs to
be updated to point to another region
    * The extra area mapped may be mapped again by Xen with different
memory attribute. This would result to attribute mismatch.

Therefore, rework the logic in create_page_tables() to map only what's
necessary. To simplify the logic, we also want to make sure _end
is page-aligned. So align the symbol in the linker and add an assert
to catch any change.

Lastly, take the opportunity to confirm that _start is equal to
XEN_VIRT_START as the assembly is using both interchangeably.

Both the stub and the x86 prototypes for replace_grant_host_mapping()

While there are no differences for 64-bit, for 32-bit it means

Julien Grall c4b72c4a54 xen/arm64: mm: Rework switch_ttbr()
At the moment, switch_ttbr() is switching the TTBR whilst the MMU is
still on.

Switching TTBR is like replacing existing mappings with new ones. So
we need to follow the break-before-make sequence.

In this case, it means the MMU needs to be switched off while the
TTBR is updated. In order to disable the MMU, we need to first
jump to an identity mapping.

Rename switch_ttbr() to switch_ttbr_id() and create an helper on
top to temporary map the identity mapping and call switch_ttbr()
via the identity address.

switch_ttbr_id() is now reworked to temporarily turn off the MMU
before updating the TTBR.

We also need to make sure the helper switch_ttbr() is part of the
identity mapping. So move _end_boot past it.

The arm32 code will use a different approach. So this issue is for now
only resolved on arm64.

Julien Grall 1c78d76b67 xen/arm64: mm: Introduce helpers to prepare/enable/disable the identity mapping
In follow-up patches we will need to have part of Xen identity mapped in
order to safely switch the TTBR.

On some platform, the identity mapping may have to start at 0. If we always
keep the identity region mapped, NULL pointer dereference would lead to
access to valid mapping.

It would be possible to relocate Xen to avoid clashing with address 0.
However the identity mapping is only meant to be used in very limited
places. Therefore it would be better to keep the identity region invalid
for most of the time.

Two new external helpers are introduced:
    - arch_setup_page_tables() will setup the page-tables so it is
easy to create the mapping afterwards.
    - update_identity_mapping() will create/remove the identity mapping

Julien Grall 9d267c049d xen/arm64: Rework the memory layout
Xen is currently not fully compliant with the Arm Arm because it will
switch the TTBR with the MMU on.

In order to be compliant, we need to disable the MMU before
switching the TTBR. The implication is the page-tables should
contain an identity mapping of the code switching the TTBR.

In most of the case we expect Xen to be loaded in low memory. I am aware
of one platform (i.e AMD Seattle) where the memory start above 512GB.
To give us some slack, consider that Xen may be loaded in the first 2TB
of the physical address space.

The memory layout is reshuffled to keep the first four slots of the zeroeth
level free. All the regions currently in L0 slot 0 will not be part of
slot 4 (2TB). This requires a slight tweak of the boot code because
XEN_VIRT_START (2TB + 2MB) cannot be used as an immediate.

This reshuffle will make trivial to create a 1:1 mapping when Xen is
loaded below 2TB.

Lastly, take the opportunity to check at compile time if any of the
regions may overlap with the reserved area for identity mapping.

and it will be convenient to use the fixmap area for accessing

Julien Grall fbd9b5fb4c xen/arm32: head: Remove restriction where to load Xen
At the moment, bootloaders can load Xen anywhere in memory but the
region 2MB - 4MB. While I am not aware of any issue, we have no way
to tell the bootloader to avoid that region.

In addition to that, in the future, Xen may grow over 2MB if we
enable feature like UBSAN or GCOV. To avoid widening the restriction
on the load address, it would be better to get rid of it.

When the identity mapping is clashing with the Xen runtime mapping,
we need an extra indirection to be able to replace the identity
mapping with the Xen runtime mapping.

Reserve a new memory region that will be used to temporarily map Xen.
For convenience, the new area is re-using the same first slot as the
domheap which is used for per-cpu temporary mapping after a CPU has
booted.

Furthermore, directly map boot_second (which cover Xen and more)
to the temporary area. This will avoid to allocate an extra page-table
for the second-level and will helpful for follow-up patches (we will
want to use the fixmap whilst in the temporary mapping).

Lastly, some part of the code now needs to know whether the temporary
mapping was created. So reserve r12 to store this information.

Julien Grall 5d71766bd1 tools/xenstore: Fail a transaction if it is not possible to create a node
Commit f2bebf72c4 "xenstore: rework of transaction handling" moved
out from copying the entire database everytime a new transaction is
opened to track the list of nodes changed.

The content of all the nodes accessed during a transaction will be
temporarily stored in TDB using a different key.

The function create_node() may write/update multiple nodes if the child
doesn't exist. In case of a failure, the function will revert any
changes (this include any update to TDB). Unfortunately, the function
which reverts the changes (i.e. destroy_node()) will not use the correct
key to delete any update or even request the transaction to fail.

This means that if a client decide to go ahead with committing the
transaction, orphan nodes will be created because they were not linked
to an existing node (create_node() will write the nodes backwards).

Once some nodes have been partially updated in a transaction, it is not
easily possible to undo any changes. So rather than continuing and hit
weird issue while committing, it is much saner to fail the transaction.

This will have an impact on any client that decides to commit even if it
can't write a node. Although, it is not clear why a normal client would
want to do that...

Lastly, update destroy_node() to use the correct key for deleting the
node. Rather than recreating it (this will allocate memory and
therefore fail), stash the key in the structure node.

the destructors in case of success.

Julien Grall 8a2111250b xen/x86: p2m: Add preemption in p2m_teardown()
The list p2m->pages contain all the pages used by the P2M. On large
instance this can be quite large and the time spent to call
d->arch.paging.free_page() will take more than 1ms for a 80GB guest
on a Xen running in nested environment on a c5.metal.

By extrapolation, it would take > 100ms for a 8TB guest (what we
current security support). So add some preemption in p2m_teardown()
and propagate to the callers. Note there are 3 places where
the preemption is not enabled:
    - hap_final_teardown()/shadow_final_teardown(): We are
preventing update the P2M once the domain is dying (so
no more pages could be allocated) and most of the P2M pages
will be freed in preemptive manneer when relinquishing the
resources. So this is fine to disable preemption.
    - shadow_enable(): This is fine because it will undo the allocation
that may have been made by p2m_alloc_table() (so only the root
      page table).

The preemption is arbitrarily checked every 1024 iterations.

We now need to include <xen/event.h> in p2m-basic in order to
import the definition for local_events_need_delivery() used by
general_preempt_check(). Ideally, the inclusion should happen in
xen/sched.h but it opened a can of worms.

Note that with the current approach, Xen doesn't keep track on whether
the alt/nested P2Ms have been cleared. So there are some redundant work.
However, this is not expected to incurr too much overhead (the P2M lock
shouldn't be contended during teardown). So this is optimization is
left outside of the security event.

At the moment the P2M page tables will be freed when the domain structure
is freed without any preemption. As the P2M is quite large, iterating
through this may take more time than it is reasonable without intermediate
preemption (to run softirqs and perhaps scheduler).

Split p2m_teardown() in two parts: one preemptible and called when
relinquishing the resources, the other one non-preemptible and called
when freeing the domain structure.

As we are now freeing the P2M pages early, we also need to prevent
further allocation if someone call p2m_set_entry() past p2m_teardown()
(I wasn't able to prove this will never happen). This is done by
the checking domain->is_dying from previous patch in p2m_set_entry().

Similarly, we want to make sure that no-one can accessed the free
pages. Therefore the root is cleared before freeing pages.

Since commit 01280dc19c "evtchn: simplify port_is_valid()", the event
channels code assumes that all the buckets below d->valid_evtchns are
always allocated.

This assumption hold in most of the situation because a guest is not
allowed to chose the port. Instead, it will be the first free from port
0.

When static event channel support will be added for dom0less domains
user can request to allocate the evtchn port numbers that are scattered
in nature.

The existing implementation of evtchn_allocate_port() is not able to
deal with such situation and will end up to override bucket or/and leave
some bucket unallocated. The latter will result to a droplet crash if
the event channel belongs to an unallocated bucket.

This can be solved by making sure that all the buckets below
d->valid_evtchns are allocated. There should be no impact for most of
the situation but LM/LU as only one bucket would be allocated. For
LM/LU, we may end up to allocate multiple buckets if ports in use are
sparse.

A potential alternative is to check that the bucket is valid in
is_port_valid(). This should still possible to do it without taking
per-domain lock but will result a couple more of memory access.

(XEN) 2ND[0x1f] = 0x00500000bfe00f7d

The content of the entries are not aligned making a bit trickier to
read (I appreciate this is a matter of taste).

Align the values by forcing the index to be always printed using
3 characters (enough to cover 512 in hexadecimal).

New output:

(XEN) 1ST[0x001] = 0x000000004015ff7f
(XEN) 2ND[0x01f] = 0x00500000bfe00f7d

Julien Grall 3babad2af8 xen/arm32: head: Move earlyprintk messages to .rodata.str
At the moment, the strings are in text right after each use because
the instruction 'adr' has specific requirement on the location
and the compiler will forbid cross section label.

The macro 'adr_l' was recently reworked so the caller doesn't need
to know whether the MMU is on. This makes it easier to use where
instructions can be run in both context.

This also means that the strings don't need to be part of .text
anymore. So move them to .rodata.str.

Julien Grall a571c1aab5 xen/arm32: heap: Rework adr_l so it doesn't rely on where Xen is loaded
At the moment, the macro addr_l needs to know whether the caller
is running with the MMU on. This is fine today because there are
only two possible cases:
 1) MMU off
 2) MMU on and linked to the virtual address

This is still cumbersome to use for the developer as they need
to know if the MMU is on.

Thankfully, Linux developpers came up with a great way to allow
adr_l to work within the range +/- 4GB of PC by emitting a PC-relative
reference [1].

Re-use the same approach on Arm and drop the parameter 'mmu'.

Julien Grall f93f7f70de xen/arm32: head: Introduce get_table_slot() and use it
There are a few places in the code that need to find the slot at a
given page-table level.

So create a new macro get_table_slot() for that. This will reduce
the effort to figure out whether the code is doing the right thing.

The new macro is using 'ubfx' (or 'lsr' for the first level) rather
than the existing sequence (mov_w, lsr, and) because it doesn't require
a scratch register and reduce the number of instructions (4 -> 1).

There are a few places in the code that need to find the slot
at a given page-table level.

So create a new macro get_table_slot() for that. This will reduce
the effort to figure out whether the code is doing the right thing.

Take the opportunity to use 'ubfx'. The only benefits is reducing
the number of instructions from 2 to 1.

The new macro is used everywhere we need to compute the slot. This
requires to tweak the parameter of create_table_entry() to pass
a level rather than shift.

Note, for slot 0 the code is currently skipping the masking part. While
this is fine, it is safer to mask it as technically slot 0 only covers
bit 48 - 39 bit (assuming 4KB page granularity).

Take the opportunity to correct the comment when finding the second
slot for the identity mapping (we are computing the second slot
rather than first).

At the moment, xen_second is used to cover the first 2GB of the
virtual address space. With the recent rework of the page-tables,
only the first 1GB region (where Xen resides) is effectively used.

In addition to that, I would like to reshuffle the memory layout.
So Xen mappings may not be anymore in the first 2GB of the virtual
address space.

Therefore, rework xen_second so it only covers the 1GB region where
Xen will reside.

With this change, xen_second doesn't cover anymore the xenheap area
on arm32. So, we first need to add memory to the boot allocator before
setting up the xenheap mappings.
