Roger Pau Monne 8845155c83 pvshim: make PV shim build selectable from configure
So a user can decide whether to compile a PV shim as part of the tools
build. Note that the default behavior is preserved, which is to build
a PV shim when the target or host (if target is unset) architecture is
64bit x86.

Roger Pau Monné 6fa1755644 amd/npt/shadow: replace assert that prevents creating 2M/1G MMIO entries
The assert was originally added to make sure that higher order
regions (> PAGE_ORDER_4K) could not be used to bypass the
mmio_ro_ranges check performed by p2m_type_to_flags.

This however is already checked in set_mmio_p2m_entry, which makes
sure that higher order mappings don't overlap with mmio_ro_ranges,
thus allowing the creation of high order MMIO mappings safely.

Replace the assert to allow 2M/1G entries to be created for MMIO
regions and add some extra asserts as a replacement to make sure
there's no overlapping with MMIO read-only ranges.

Note that 1G MMIO entries will not be created unless mmio_order is
changed to allow it.

Roger Pau Monne e5268a3ce3 x86/mm-locks: apply a bias to lock levels for control domain
paging_log_dirty_op function takes mm locks from a subject domain and
then attempts to perform copy to operations against the caller domain
in order to copy the result of the hypercall into the caller provided
buffer.

This works fine when the caller is a non-paging domain, but triggers a
lock order panic when the caller is a paging domain due to the fact
that at the point where the copy to operation is performed the subject
domain paging lock is locked, and the copy operation requires
locking the caller p2m lock which has a lower level.

Fix this limitation by adding a bias to the level of control domain mm
locks, so that the lower control domain mm lock always has a level
greater than the higher unprivileged domain lock level. This allows
locking the subject domain mm locks and then locking the control
domain mm locks, while keeping the same lock ordering and the changes
mostly confined to mm-locks.h.

Note that so far only this flow (locking a subject domain locks and
then the control domain ones) has been identified, but not all
possible code paths have been inspected. Hence this solution attempts
to be a non-intrusive fix for the problem at hand, without discarding
further changes in the future if other valid code paths are found that
require more complex lock level ordering.

Current logic to handle long running operations is flawed because it
doesn't prevent the guest vcpu from running. Fix this by raising a

call in the guest entry path performs a rescheduling. Also move the
call to vpci_process_pending into handle_hvm_io_completion, together
with the IOREQ code that handles pending IO instructions.

Note that a scheduler softirq is also raised when the long running

Fix this by writing the command register once the guest physmap

Flush checking for AMD IOMMU didn't check whether the previous entry
was present, or whether the flags (writable/readable) changed in order
to decide whether a flush should be executed.

Fix this by taking the writable/readable/next-level fields into account,
together with the present bit.

Along these lines the flushing in amd_iommu_map_page() must not be
omitted for PV domains. The comment there was simply wrong: Mappings may
very well change, both their addresses and their permissions. Ultimately
this should honor iommu_dont_flush_iotlb, but to achieve this
amd_iommu_ops first needs to gain an .iotlb_flush hook.

Also make clear_iommu_pte_present() static, to demonstrate there's no
caller omitting the (subsequent) flush.

This is part of XSA-275.

Several people have reported hardware issues (malfunctioning USB
controllers) due to iommu page faults on Intel hardware. Those faults
are caused by missing RMRR (VTd) entries in the ACPI tables. Those can
be worked around on VTd hardware by manually adding RMRR entries on
the command line, this is however limited to Intel hardware and quite
cumbersome to do.

In order to solve those issues add a new dom0-iommu=map-reserved
option that identity maps all regions marked as reserved in the memory
map. Note that regions used by devices emulated by Xen (LAPIC, IO-APIC
or PCIe MCFG regions) are specifically avoided. Note that this option
is available to all Dom0 modes (as opposed to the inclusive option
which only works for PV Dom0).

Roger Pau Monné 09b3907f93 The hvmloader binary generated when using LLVM LD doesn't work
properly and seems to get stuck while trying to generate and load the
ACPI tables. This is caused by the layout of the binary when linked
with LLVM LD.

LLVM LD has a different default linker script that GNU LD, and the
resulting hvmloader binary is slightly different:

LLVM LD:
Program Headers:
Type Offset VirtAddr PhysAddr FileSiz MemSiz Flg Align
  PHDR 0x000034 0x000ff034 0x000ff034 0x00060 0x00060 R 0x4
  LOAD 0x000000 0x000ff000 0x000ff000 0x38000 0x38000 RWE 0x1000
  GNU_STACK 0x000000 0x00000000 0x00000000 0x00000 0x00000 RW 0

GNU LD:
Program Headers:
Type Offset VirtAddr PhysAddr FileSiz MemSiz Flg Align
  LOAD 0x000080 0x00100000 0x00100000 0x36308 0x3fd74 RWE 0x10
  GNU_STACK 0x000000 0x00000000 0x00000000 0x00000 0x00000 RW 0x4

Note that in the LLVM LD case (as with GNU LD) the .text section does
indeed have the address set to 0x100000 as requested on the command
line:

[ 1] .text PROGBITS 00100000 001000 00dd10 00 AX 0 0 16

There's however the PHDR which is not present when using GNU LD.

Fix this by using a very simple linker script that generates the same
binary regardless of whether LLVM or GNU LD is used. By using a linker

Roger Pau Monné 18cd4997d2 x86/efi: move the logic to detect PE build support
So that it can be used by other components apart from the efi specific
code. By moving the detection code creating a dummy efi/disabled file
can be avoided.

This is required so that the conditional used to define the efi symbol
in the linker script can be removed and instead the definition of the
efi symbol can be guarded using the preprocessor.

The motivation behind this change is to be able to build Xen using lld
(the LLVM linker), that at least on version 6.0.0 doesn't work
properly with a DEFINED being used in a conditional expression:

ld -melf_x86_64_fbsd -T xen.lds -N prelink.o --build-id=sha1 \

Roger Pau Monné be07023be1 x86/vhpet: add support for level triggered interrupts
Level triggered interrupts are not an optional feature of HPET, and
must be implemented in order to comply with the HPET specification.

Implement them by adding a callback to the timer which sets the
interrupt bit in the general interrupt status register. Further
interrupts (in case of periodic mode) will not be injected until the
bit is cleared.

In order to reset the interrupts when the status bit is clear Xen must
also detect accesses to such register.

While there convert tn and i in hpet_write to unsigned.

Add a new 'level' parameter to create_periodic_time in order to create

Note that periodic level triggered interrupts are not supported. This
is because level triggered interrupts always require a deassert of the
IO-APIC pin, which should be done by the caller of vpt at which point
the caller should also reset the timer if required.

Roger Pau Monné 1e34ed7174 vpci/msi: fix update of bound MSI interrupts
Current update process of already bound MSI interrupts is wrong
because unmap_domain_pirq calls pci_disable_msi, which disables MSI
interrupts on the device. On the other hand map_domain_pirq doesn't
enable MSI, so the current update process of already enabled MSI
entries is wrong because MSI control bit will be disabled by
unmap_domain_pirq and not re-enabled by map_domain_pirq.

In order to fix this avoid unmapping the PIRQs and just update the
binding of the PIRQ. A new arch helper to do that is introduced.

Note that MSI-X is not affected because unmap_domain_pirq only
disables the MSI enable control bit for the MSI case, for MSI-X the
bit is left untouched by unmap_domain_pirq.
